| 摘 要： | 目的总结腰-硬膜外联合麻醉在老年择期手术中的应用效果。方法选择80例行腹部和下肢择期手术的老年患者,年龄在58~82岁之间,分为CSEA组和EA组。每组各40例,两组患者的术前准备和术前用药相同。CSEA组根据术中腰麻作用减退情况加用局麻药;EA组在确认出现节段性硬膜外阻滞平面排除全脊髓麻醉,按需追加局麻药物。结果 CSEA组麻醉诱导时间明显短于EA组,硬膜外局麻用药剂量明显少于EA组;麻醉效果:CSEA组40例中,优36例,明显多于EA组的28例,而且EA组中有4例麻醉效果欠佳,改为全麻。结论腰-硬膜外联合麻醉,具有起效迅速、作用完善和作用时间点灵活的双重优点,且对硬膜损伤小,值得临床广泛开展。

| Abstract: | Objective To summarize the application of lumbar and epidural anesthesia for senile patient.Methods 80 cases of seniel patient who underwent abdominal and lower limb operation age from 58~82 were divided into CSEA group and EA group.The drug used before operation was same.The additional drug was used during operation depanding the situation of anesthesia.Results The induced time and local drug used of CSEA were less that those in EA group.The anesthesia effect of CSEA group was better than that of EA group in which 4 cases were underwent to general anesthesia as bad effect.Conclusion The CSEA might have rapid,complete and levis action time,less injury of dura that should be applied in clinic. |
