单肺移植中支气管吻合技术的应用总结

目的:探讨单肺移植手术支气管吻合的技术经验。方法: 2003年1月∼2008年5月其间，以改良套入式吻合法(膜部连续+软骨部交替间断褥式和8字缝合)施行了同种异体单肺移植手术13例，其中右侧9例，左侧4例。回顾性分析了该组患者的支气管吻合口并发症发生情况。结果：所有患者都度过围手术期（>30d），1例患者在术后22天死于原发性移植肺功能不全，2例患者分别于术后74天和77天死于自体肺发生自发性气胸导致心跳骤停并最终引发的于肺炎。所有患者均未曾发生吻合口瘘，1例（7.7%）患者出现支气管吻合口狭窄，予以支架处理后效果满意。结论：改良套入式吻合法是肺移植可靠的支气管吻合技术。

The technique for bronchial anastomoses in single lung transplantation

Objective To report the experience on the bronchial anastomosis for lung transplantation in a single center. Methods From January 2003 and May 2008, 13 single lung transplantations with a modified bronchial anastomosis technique were performed. Complications associated with postoperative bronchial anastomosis were reviewed. Results all the cases survived more than day 30 postoperatively. One case died of primary graft dysfunction on day 22 postoperatively; two cases died of pneumonia secondary to cardiac arrest due to spontaneous pneumothorax on postoperative day 74，77 respectively. No dehiscence occurred and 1 of 13 cases experienced postoperative stenosis of bronchial anastomosis successfully treated by stent. Conclusions the modified bronchial anastomosis technique is reliable for lung transplantion.
