50％笑气吸入用于分娩镇痛对母儿影响的调查

目的观察50％笑气吸入分娩镇痛对母儿的影响。方法将121例产妇在宫口开全3cm开始使用50％的笑气吸入镇痛设为镇痛组,选择125例产妇在分娩过程中不使用任何镇痛方法作为对照组,应用焦虑自评量表(SAS)测评两组产妇的情绪状态,观测比较两组的产痛程度、总产程时间、分娩方式、产时失血量、新生儿窒息率情况。结果分娩结束后2 h评价两组SAS评分情况,镇痛组SAS评分低于对照组,均P<0．01;产痛程度比较,宫口开大3 cm时镇痛组高于对照组,均P<0．01,宫口近开全时镇痛组疼痛程度低于对照组,均P<0．01;镇痛组选择剖宫产率、总产程时间较对照组降低, 两组比较,均P<0．01。结论笑气吸入镇痛分娩,可缩短产程时间,对母儿无不良影响,有利于产妇保持分娩中的良好心态。

Effect of the inhaling 50 % laughing gas for labor pain relief on maternal and fetus

Objective To investigate the effect of the inhalation 50% laughing gas on labor pain relief. Methods A total 121 cases of pregnant women were provided with 50% laughing gas (pain ease group). Another 125 cases did not use any labor pain relief in labor courses (control group). Mental condition in two groups of pregnant women were determined and reviewed with self-rating anxiety Scale (SAS) and self-rating depression (SDS). The degree of labor pains, quantity of postpartum hemorrhage, and rate of birth asphyxia were compared between the two groups. Results Before labor the SAS and SDS in the ease pain group was higher than in the control group, P < 0.01; when ostium uteri outspread to 3 cm, degree of pains in the ease pain group was higher than the control group in pregnant women P < 0.01; but when ostium outspread to 9 cm, the result was just the reverse, P < 0.01;significant differences were found in the delivery process, P < 0.01. Conclusions Analgesic labor with inhaling laughing gas produces a shorter delivery process without any side effect on both mother and fetus, and keeps a good mental condition. The inhalation of nitrous oxide is a safe, effective and easy method.
