双源CT与超声心动图在儿童先天性心脏病诊断中的应用价值

摘    要:目的通过双源CT(dual source spiral computed tomography,DSCT)及超声心动图(Echocardiogram,ECG)与手术结果的对比分析,探讨两者对儿童先天性心脏病(先心病)的诊断价值。方法对35例先心病患儿的DSCT和ECG资料进行回顾性分析,并与手术结果对照。全部病例均经手术证实。结果35例患儿中手术证实畸形:119处,DSCT与ECG诊断的准确率分别为90.76%和87.39%,两者差异无统计学意义。其中,心内畸形56处,双源CT漏误诊8处,ECG漏误诊2处,两种检查的诊断准确率差异有统计学意义。心外畸形63处,双源CT漏误诊3处,ECG漏误诊13处,两种检查的诊断准确率差异有统计学意义。结论DSCT在心外血管结构的显示上较ECG有优势,而ECG对心内结构的评价更准确,DSCT与ECG联合应用可提高对儿童先心病的诊断准确率。

Abstract:Objective To discuss the clinical value of dual source spiral computed tomography (DSCT) and echocardiogram (ECG) in the assessment of deformities in patients with child congenital heart disease. Methods Thirty- five preoperative patients'imaging data of DSCT and ECG were analyzed retrospectively. All cases were confirmed by surgery. The results of imaging diagnosis were compared with that of surgery. Results Among 35 preoperative patients, there were 119 deformities confirmed by surgery. The diagnostic accuracy of DSCT and ECG was 90.76% and 87.39% respectively. There was no statistical significance between DSCT and ECG in diagnostic accuracy. There were 56 intracardiac deformities,8 missed by DSCT and 2 missed by ECG. ECG was superior to DSCT in the detection of intra-cardiac deformities. There were 63 extra-cardiac deformities,3 missed by DSCT and 13 missed by ECG. DSCT was superior to ECG in the detection of extra-cardiac deformities. Conclusion DSCT is superior to ECG in detection of extra-cardiac a- nomalies, while ECG is superior to DSCT in detection of intra-cardiac anomalies. The diagnostic accuracy of child congenital heart disease will be improved with application of DSCT combined with ECG.
