主动脉内球囊反搏辅助治疗急性心肌梗死并泵衰竭35例

目的探讨主动脉内球囊反搏(IABP)在急性心肌梗死(AMI)并泵衰竭患者经皮冠状动脉介入(PCI)治疗的临床疗效。方法观察组35例急性心肌梗死并泵衰竭患者在IABP辅助下行冠脉造影及PCI术。对照组24例AMI并泵衰竭患者仅使用常规药物及PCI术。观察组PCI术前或术后置入IABP球囊。结果观察组使用IABP后低血压或休克状况明显好转、尿量明显增多、心率明显减慢30例(85.71%),死亡5例(14.29%)。对照组24例中,患者血液动力学得以改善并接受PCI术17例(70.83%),死亡7例(29.17%)。两组疗效比较有显著性差异(P<0.01)。结论IABP辅助治疗下介入治疗急性心肌梗死并泵衰竭患者,可明显降低其心源性死亡率,改善生存率。

Intra aortic balloon pump in adjuvant treatment of 35 patients with acute myocardial infarction complicated with pump failure

Objective To investigate the efficacy of combination of intra aortic balloon pump (IABP) and percutaneous coronary intervention(PCI)for treatment of acute myocardial infarction(AMI) complicated with pump failure.Methods Thirty-five AMI patients complicated with pump failure were performed with coronary arteriography and PCI with accessory IABP in observative group.Twenty-four cases of control group were performed with routine medication and PCI.Before and after PCI,the balloon of IABP was placed into aorta in observative group.Results After IABP,in observative group 30 cases(85.7%) were successful including improvement of status of hypotension ,increment of amount of urine and obvious reduction of heat rate.Five cases(14.2%) were dead.Among 24 cases of control group ,hemodynamics was improved and accepted PCI in 17 cases (70.83%),7 cases (29.17%)were dead .There was obvious difference in effectiveness in two groups(P<0.01).Conclusion IABP in adjuvant treatment of AMI patients with pump failure can reduce obviously cardiogenic mortality and improve survival rate.
