两种方法治疗肱骨近端骨折并肩关节前脱位对比研究

目的：观察两种方法治疗肱骨近端骨折并肩关节前脱位的效果。方法：选择1999年7月-2005年1月在我院住院治疗的138例肱骨近端骨折并肩关节前脱位患者,按治疗方法分为经皮导入内固定组（81例）及切开复位LCP钢板内固定组（57例）,采用回顾性研究方法对两种治疗方法的手术时间、骨折愈合时间、术后并发症及肩关节功能评分进行对比研究。结果：两种治疗方在手术时间、骨折愈合时间、术后并发症及肩关节功能评分方面经统计学分析,其差异有统计学意义。结论：经皮导入内固定方法治疗肱骨近端骨折并肩关节前脱位具有操作简便、骨折愈合与肩关节稳定时间短、并发症与后遗症少、肩关节功能恢复好等优点,是目前治疗该类型损伤较为理想的方法。

A Comparative Study of Two Methods in Treating Proximal Humerus Fracture with Anterior Dislocation of Shoulder

Objective： To observe the clinical effects of two methods in treatment of proximal humerus fracture with anterior dislocation of shoulder. Methods： From July 1999 to January 2005, 138 cases of proximal humerus fracture with anterior dislocation of shoulder were treated respectively with percutaneous imported internal fixation （PIIF） （81 cases） or open re duction and internal fixation with locking compression plate （LCP） （57 cases）. All patients were retrospectively studied and statistically analyzed. Results： There were significant differences in operation time, fracture healing time, postoperative complications and shoulder function of the patients between the two methods. Conclusion： Treatment of proximal humerus fracture with anterior dislocation of shoulder using PIIF is a simple operation and able to shorten fracture healing time, reduce complications and sequelae and provide better joint function recovery, which is an ideal treatment for this type of injury.
