壳聚糖止血敷料用于经桡动脉冠状动脉介入治疗术后压迫止血的临床观察

摘    要:目的探讨壳聚糖止血敷料在经桡动脉冠状动脉介入治疗术后止血、预防穿刺血管并发症等方面的临床应用效果。方法 2010年8~10月,连续入选行经桡动脉冠状动脉介入治疗的患者220例,按照完全随机设计分入壳聚糖止血敷料加压止血组(壳聚糖组)、桡动脉专用止血器组(对照组),每组各110例。主要研究终点为住院期间术后桡动脉血管并发症,次要研究终点包括桡动脉压迫止血时间、患者舒适度。结果壳聚糖组患者术后血管并发症总发生率低于对照组(3.6%比11.8%,P=0.023)。壳聚糖组压迫止血总时间较对照组短(222.5±46.1)min比(291.5±45.6)min,P<0.001],术后患者舒适度壳聚糖组优于对照组。结论在经桡动脉介入治疗术后,应用壳聚糖止血敷料进行穿刺点的压迫止血减少了术后血管并发症的发生且可显著缩短止血操作时间。同时,提高患者舒适度。

Abstract:Objective To evaluate the hemostatic properties and efficacy of chitosan dressings in patients undergoing transradial coronary intervention. Methods From August 2010 to October 2010, 220 patients with coronary artery disease underwent transradial coronary intervention were enrolled and randomly divided into chitosan dressings compression group and control group (110 in each group). The primary study endpoints were in-hospital vascular related complications. The secondary endpoints included: compression time and the comfort feeling of patients. Results The overall incidence of vascular related complications was significantly lower in chitosan group than in control group ( 3.60/0 vs. 11.8% , P =0. 023). Compression time in ehitosan group was significantly shorter than in control group (222. 5 ± 46. 1 ) min vs. (291.5 ± 45.6) rain, P 〈 0. 001 ]. Patients' degree of comfort of feeling in chitosau group was superior to that in control group. Conclusions The application of chitosan dressings in transradial coronary intervention can reduce vascular related complications and compression time, and can also improve patients' degree of comfort of feeling.
