摘    要:目的 :观察不同程度切口刺激对大鼠脊髓背角Fos表达的影响。方法 :雄性Wistar大鼠 15只随机分为三组 :A组为对照组 ,不作手术切口 ;B组为皮肤筋膜切开组 ,仅切开皮肤及筋膜 ,不切割肌肉 ;C组为肌肉切割组 ,切开皮肤、筋膜并纵行切割肌肉。每组 5只。B组和C组大鼠在吸入乙醚麻醉下消毒左后肢 ,进行手术切口刺激 :从足跟近端 0 .5cm处向趾部作一长约 1cm的纵行切口 ,B组大鼠切开皮肤和筋膜后缝合皮肤 ,C组大鼠用镊子挑起足底肌肉并纵向切割。 1h后观察动物疼痛行为的改变 ,以累积疼痛评分评定疼痛行为 ,2h后用免疫组织化学方法观察脊髓背角Fos表达的变化。结果 :①与A组相比 ,B组和C组大鼠的累积疼痛评分明显上升 (P <0 .0 1) ,但两组之间无差异 (P >0 .0 5 ) ;②Fos阳性神经元主要分布于B组和C组手术侧脊髓背角Ⅰ、Ⅱ层 ,B组Fos阳性神经元的平均数目比C组明显增多 (P <0 .0 5 )。结论 :不同程度切口刺激对大鼠脊髓背角Fos表达的影响不同。

Abstract:Objective To observe the effect of different incision stimulation on the expression of Fos in the spinal dorsal horn in the rat. Methods 15 adult male Wistar rats were divided randomly into three groups (5 animals each): group A (control group); group B (skin and fascia incision group): only a skin and fascia incision was made and group C (skin, fascia and muscle incision group): an incision of skin, fascia and muscle was made. An incision of 1 cm length was made in the planter region of left hind paw under ether anesthesia. Pain behaviors were assessed using cumulative pain score 1 h after incision, 2 h later rats were deeply anesthetized and chest was opened, 100ml of normal saline was infused via left ventricle followed by 500 ml of 4% paraformaldehyde in 0.1M phosphate buffer infusion for fixation of tissue. The spinal cord of lumbar 4~5(L 4~5 ) were removed. The expression of Fos in the spinal dorsal horn was measured by immumohistochemistry technique of streptavidin biotin enzyme complex (SABC). Results ①The cumulative pain score were increased significantly after plantar incision in group B and C( P <0.01) compared with group A, but there was no difference between the two groups( P >0.05);②Fos positive neurons were distributed mainly in the incision ipsilateral laminaⅠand Ⅱ of spinal dorsal horn in group B and group C,the average numbers of Fos positive neuron in group B was higher than that of in group C ( P <0.05). Conclusion The effect of different incision stimulation on the expression of Fos in the spinal dorsal horn was different in the rat.
