| 摘 要： | 目的检测前列腺增生症（BPH）和前列腺癌（PCa）患者血清中总PSA（t-PSA）和游离PSA（f-PSA）及其f-PSA／t—PSA的比值，探讨其对PCa和HBP患者的诊断价值。方法采用微粒子酶免疫法（MEIA）检测54例BPH和48例PCa患者血清中t-PSA、f-PSA及f-PSA／t—PSA的比值。结果血清总PSA在4．0～10．0ng／ml时，BPH与PCa组的t-PSA差异无统计学意义（P〉0．05），游离PSA、f-PSA／t—PSA比值差异有统计学意义（P〈0．05）。结论当血清t-PSA超过4．0—10．0ng／ml时，f-PSA／t-PSA的比值对PCa和BPH的鉴别诊断具有重要的临床价值。以f-PSA／t-PSA〈0．16为界定的标准，特异性为88．39％，灵敏度为79．36％。在诊断BPH和PCa时，f-PSA／t-PSA明显优于t—PSA，是早期诊断PCa的指标。

| Abstract: | Objective To evaluate the f-PSA/t-PSA value in diagnosis prostate tumor by integrally testing the serum t-PSA, f-PSA as well as ratio of f-PSA to t-PSA. Methods The serum t-PSA, f-PSA of 54 benign prostate hyperplasia （ BPH ） and 48 prostate cancer（PCa） were measured by particle enzyme immunoassay（EMIA） ,to find out the ratio of f-PSA/t-PSA. Results When serum t-PSA ranged from 4.0 to 10.0 ng/ml. It is difficult to differentiate PCa from BPH by t-PSA or f-PSA alone. But when using the ratio of f-PSA/t-PSA 〈 0.16 as the diagnostic criterion,its specificity and sensitivity were 88.39% and 79.36% respectively, much higher than that of t-SPA, f-PSA （ P 〈 0.01 ）. Conclusion When plasma t-PSA in higher than normal, the ratio of f-PSA/t-PSA is of great clinical value in differentiating the prostate tumor from BPH. |
