CA125升高患者联合检测CEA在临床鉴别诊断中的价值

目的了解糖类抗原125(CA125)升高患者联合检测癌胚抗原(CEA)在临床鉴别诊断中的价值。方法对379例血清CA125水平高于参考值范围(35.0 U.mL-1)的患者进行回顾性分析,同时对其CEA水平进行比较。分为结核组124例、恶性肿瘤组244例、肺炎组11例。结果当CA125在35～200 U.mL-1时,结核组、肺炎组与肿瘤组比较差异无显著性(P0.05),肿瘤组CEA阳性率显著高于结核组和肺炎组(P0.01);当CA125≥200 U.mL-1时,恶性肿瘤组CA125高于结核组(P0.05),肿瘤组CEA阳性率显著高于结核组(P0.01)。43例结核组患者抗痨治疗1～3个月后CA125水平明显降低(P0.05)。结论 CA125,CEA联合检测有助于结核和恶性肿瘤鉴别诊断,动态观察CA125水平可作为治疗结核疗效判断指标。

Value of measurement of CA125 and CEA in differential diagnosis of tuberculous and malignant tumor

Objective To investigate the value of carbohydrate antigen 125（CA125）and carcinoembryonic antigen（CEA）in differential diagnosis of tuberculosis（TB）and malignant tumors.Methods The 379 hospitalized patients whose serum CA125 levels above the reference range（35.0 U·mL-1）were analyzed retrospectively,while CEA levels were compared.The statistics were divided into TB groups（124 cases）,malignant tumor group（244 cases）and pneumonia group（11 cases）.Results While CA125 was（35~200 U·mL-1）,the CA125 levels of TB group,tumor group and pneumonia group showed no significant difference（P0.05）.CEA levels of tumor group were significantly higher than that in TB group and pneumonia groups（P0.01）.While CA 125≥200 U·mL-1,the CA125 levels of malignant tumor group was significantly higher than those in the TB group（P0.05）,positive rate of CEA of tumor group was significantly higher than in the TB group（P0.01）.After anti-tuberculosis treatment one to three months,CA125 of 43 tuberculosis cases were significantly reduced（P0.05）.Conclusion CA125 and CEA detection can help the differential diagnosis of tuberculosis and malignancy,dynamic observation of CA125 levels may help to evaluate the prognosis of tuberculosis.
