连续性血液滤过滤器复用对血浆炎性细胞因子清除的影响

目的探讨在连续性静脉-静脉血液滤过(CVVH)中滤器复用对血浆TNF-α、IL-6清除的影响。方法将需要行CVVH的危重症患者随机分为2组:A组为首次使用透析器,B组为用过氧乙酸消毒后复用透析器(均为第1次复用),分别测定治疗开始后0,2,6 h血浆TNF-α、IL-6的浓度及0~2 h和4~6 h期间收集的滤出液中TNF-α、IL-6的浓度。结果透析开始后2 h和6 h血浆中TNF-α、IL-6的浓度2组比较无显著性差异(P>0.05);透析开始后0~2 h和4~6 h期间收集的滤出液中TNF-α、IL-6的浓度2组比较也无显著性差异(P>0.05)。结论CVVH时滤器复用仍能有效地清除血浆中炎性细胞因子,尤其是对IL-6有较强的清除能力。

Influence of continuous hemofiitration with reused dialyzer in clearing plasma inflammatory cytokines

Objective It is to discuss the influrence of continuous vein to vein hemofiltration(CVVH) with reused dialyzer in clearing plasma TNF-α and IL-6.Methods 15 critical patients been going to accept CVVH were randomly divided into two groups.In group A the dialyzer was used for the first time.In group B the dialyzer was reused firstly after sterilization by Peracetic Acid.The concentrations of plasma TNF-α and IL-6 were measured after CVVH performed for 0 h,2 h,4 h and 6 h.Then the concentrations of TNF-α and IL-6 in liquid by filtering from two duration were measured in 0-2 h and 4-6 h.Results No significant difference was found between two groups in their concentration of plasma TNF-α and IL-6 at the time of 0-2 h and 4-6 h,and in liquid by filtering from those durations(P>0.05).Conclusion The dialyzer reuse can maintain the effective clearance of inflammatory cytokines in plasma especially of IL-6.
