脊髓型颈椎病前路减压内固定手术疗效分析

目的:研究脊髓型颈椎病前路减压内固定手术的疗效及病程、病情与疗效之间的关系。方法:回顾性研究了我院2002年1月～2012年1月因脊髓型颈椎病行手术治疗的134例患者,其中男性94例,女性40例,49～72岁,平均年龄57.3岁,术后随访1年半～10年,平均7.2年。对其手术疗效,及病程、病情与疗效之间的关系进行分析结果:术后病人的JOA评分较术前均有明显提高,与术前JOA评分进行配对t检验,差别具有统计学意义(t=-12.915,P=0.000)。病程不同的两组患者疗效差别具有统计学意义(t=7.629,P=0.000),而病情严重程度不同的四组患者疗效差别无统计学意义(F=1.313,P=0.278)。结论:颈椎前路减压内固定术是目前治疗脊髓型颈椎病的一种有效手术方法。脊髓型颈椎病一经确诊,应尽早采取恰当、有效的手术方式,可取得较为满意的治疗效果。

Analysis of Clinical Efficacy in Anterior Decompression and Internal Fixation of Cervical Spondylotic Myelopathy

Objective ：To study the operation results of anterior decompression of cervical spondylotic myelopathy and relationship between disease course, degree with treatment result. Method： 134 eases with cervical spondylotic myelopathy treated by anterior decompression were reviewed.Relationship between disease course ,degree with their treatment results were analyzed. 94 males and 40 males, ranging from 49 to 72 years old （57.3years old on average）. 1.5 years to 10 years follow-up after operation, meanwhile 5.2years. Result： The JOA scores rise markedly after operation,paired t-test for dependent samples is used to estimate the dif ference, there is statistics difference of JOA scores between preoperation and postoperation （t= -12.915 P=0.000）. The treatment result between two groups of different disease course has statistics differences （t=7.629 P=0.000）, and the treatment result among four groups of different disease degree has no statistics differences （F=1.313 P=0,278）. Conclusion： Anterior decompression is an effective operative method to treat cervical spondylotic myelopathy.The op eration should be performed as soon as possible when cervical spondylotic myelopathy is diagnosed exactly and the satisfactory result will be achieved by proper operative way.
