剖宫产术后微小剂量吗啡复合布比卡因硬膜外镇痛效果观察

目的探讨剖宫产术后使用微小剂量吗啡复合布比卡因硬膜外镇痛效果。方法将224例剖宫产术后患者分为观察组112例和对照组112例,观察组使用微小剂量吗啡复合布比卡因(吗啡10mg+0.5%布比卡因25mL+生理盐水至100mL)硬膜外镇痛,对照组肌内注射哌替啶、异丙嗪(哌替啶50mg、异丙嗪25mg)镇痛,观察两组产妇的镇痛效果、阴道流血量、排气时间、开奶时间及不良反应发生等情况。结果观察组的镇痛效果明显优于对照组(P<0.01),两组阴道流血量及排气时间及开奶时间及不良反应比较差异无统计学意义(P>0.05)。结论剖宫产术后微小剂量吗啡复合布比卡因硬膜外自控镇痛,可达到充分镇痛的目的,不良反应小。

The Effect of Patient-controlled Epidural Analgesia (PCEA) with Minimal Dose of Morphine and Bupivacaine after Cesarean Section

Objective To approaeh the effect of Patient-eontrolled epidural analgesia（PCEA） with minimal dose of Morphine and Bupivaeaine after cesarean section. Mehtods 224 patients after cesarean section were randomly divided into two groups ： the observation group （ n = 112 ） and the eontrol group （ n = 112 ）, and the observation group had epidural analgesia （Morphine 10 mg ＋ 0.5% Bupivaeaine25 mL ＋ NS to 100 mL）, the control group were intramuscular injected with pethidin and proazamine（ pethidin 50mg、proazamine 25mg） ,observe the analgesia effect, the amount of vaginal bleeding, exhaust time, the time the mother secretes milk after birth and the side effect between these two groups. Results Th analgesia effect of the observation group were better than that of the control group（ P 〈 0.01 ） ,there was no statistical significance between the amount of vaginal bleeding and the exhaust time and the time the mother secretes milk after birth and the side effeet（P 〉 0. 05 ）. Conclusion Patient-controlled epidural analgesia （PCEA） with minimal dose of Morphine and Bupivacaine after cesarean section can provided fully pain relief,and there was less side effect.
