老年高血压患者动脉粥样硬化性肾动脉狭窄的发生率及其相关危险因素

摘    要:目的对疑似冠心病或急性、慢性心肌梗死的老年高血压患者进行冠状动脉造影及肾动脉造影,分析动脉粥样硬化性肾动脉狭窄的发生率及其相关危险因素。方法对108例疑似冠心病或急性、慢性心肌梗死接受冠状动脉造影的老年高血压患者进行肾动脉血管造影检查。结果在108例老年高血压受检者中,ARAS 42例(38.9%)。在75例老年高血压合并CAD患者中,ARAS 33例(44.0%);33例老年高血压无CAD患者中,ARAS 9例(27.3%)。冠脉多支病变发生肾动脉狭窄的机率增加,多因素Logistic回归分析表明,高血压病程延长、宽PP、高胆固醇、高肌酐水平是老年动脉粥样硬化性肾动脉狭窄的预测因素。结论对冠心病合并高血压的老年患者,尤其合并长高血压病程、宽PP、高胆固醇、高肌酐水平者,冠状动脉造影时应考虑进行腹主动脉造影检查,以尽早发现动脉粥样硬化性肾动脉狭窄。
