细胞周期素D1和p27kip1在肾癌中的表达及其意义

摘    要:目的:探讨cyclin D1、p27kip1在普通型肾细胞癌(conventional renal cell carcinoma,CRCC)发生、发展中的作用。方法:用定量RT-PCR和Western-blot方法检测25例CRCC组织和10例肿瘤远端的正常肾皮质中cyclin D1和p27kip1的mRNA和蛋白的含量;用免疫组化方法检测76例CRCC中cyclin D1、p27kip1的表达;分析cyclin D1、p27kip1的表达与肿瘤分期、分级及术后生存率的相关性。结果:CRCC中cyclin D1的mRNA和蛋白含量分别为0.488±0.399和1.069±0.371,高于正常对照组0.089±0.066和0.281±0.253(P均<0.01);cyclin D1的表达与肿瘤体积大小有关,体积大者cyclin D1高表达(P<0.05);CRCC中p27kip1mRNA和蛋白含量分别为0.191±0.111和0.146±0.051,低于正常对照组0.374±0.216和1.004±0.318(P均<0.01),随着p27kip1表达的降低,肿瘤的细胞核Fuhrman分级、TNM分期增高;p27kip1阳性组患者的5年生存率为84.26%,高于p27kip1阴性组25.25%(P<0.01)。结论:cyclin D1高表达和p27kip1的低表达与CRCC的发生有关;p27kip1的低表达可能促进肿瘤的演进,检测p27kip1可作为评价CRCC预后的参考指标。

Abstract:Objective:To investigate the role of cyclin D1 and p27kip1 in the occurrence and development of conventional renal cell carcinoma(CRCC).Methods: RT-PCR and Western-blot were used to detect mRNA and protein contents of cyclin D1 and p27kip1 in 25 CRCCs and 10 normal renal tissue distant to tumor.Immunohistochemistry was used to investigate the expression of cyclin D1 and p27kip1 in pathological tissue sections of 76 CRCCs.The relationship between those index and clinicopathological parameters was analyzed statistically.Results: In CRCC,the expression of cyclin D1 was higher than that of the control group.The higher cyclin D1 content was related to big tumor size(P<0.05);The expression of p27kip1was lower than that of the control group,and the lower p27kip1was related to higher nuclear grade and TNM stage(P<0.01).The 5-year cumulative survival rate of the p27kip1high expression group is longer than that of the low group(P<0.01).Conclusions: The excessive expression of cyclin D1 and lower expression of p27kip1play an important role in the carcinogenesis of CRCC.The lower expression of p27kip1 may affect the progression of the tumors.The detection of p27kip1 may be as a reference marker in the prognosis of CRCC.
