多普勒组织成像评价扩张型心肌病患者右心室收缩功能

目的探讨多普勒组织成像（DTI）评价扩张型心肌病（DCM）患者右心室收缩功能的价值。方法应用DTI检测了28例DCM患者和30例正常人三尖瓣环处射血期波（S）峰值速度（Vs）、速度时间积分（VTIs）、持续时间（Ts）、心电图Q波至S波起点处间期（Q—S）等，并与二维超声心动图所测三切面平均右心室面积变化率（MRVPCA）、胸骨旁左心长轴切面所测右心室前后径（RVD）等对比分析。结果DCM患者三尖瓣环处Vs、VTIs均较对照组明显减低（P〈0．05），Q—S间期延长（P〈0.01），Q—S／Ts比值增大（P〈0．01），MRVPCA减小（P〈0．01），RVD增大（P〈0．01），右心室面积两组无明显差异（P〉0．05）。结论DCM患者右心室收缩功能存在一定程度减退，应用三尖瓣环处DTI所测收缩期运动指标，可对其进行正确评价。

Doppler Tissue Imaging Evaluation of Right Ventricular Systolic Function on Patients with Dilated Cardiomyopathy

Objective To explore the value of Doppler tissue imaging(DTI)on evaluating right ventricular systolic function in patients with dilated cardiomyopathy(DCM).Methods The tricuspid annular systolic wave(S)velocities(Vs),velocity time integrals(VTIs),times of S waves(Ts),and the times from the electrocardicgaraphic Q wave to the onset of S wave(Q-S)were measured with DTI in 28 patients with DCM and 30 normal people and compared with mean right ventricular percent changes in area(MRVPCA)of three different views,as well as right ventricular antero-posterior diameter on parasternal long-axis veiw on two-dimensional echocardiography.Results Vs and VTIs of the tricuspid annular motion on DTI were significantly decreased in patients with DCM than those in the normal group(P<0.05).The Q-S was significantly longer in the DCM group than in the control group(P<0.01),The ratio of Q-S/Ts was significantly increased(P<0.01).MRVPCA was signficantly lower in DCM group than in the control group(P<0.01),RVD was signficantly bigger in DCM group than in control group(P<0.01).Conclusions Right ventricular systolic function is somewhat impaired in patients with DCM,which could be assessed accurately by the indices of the tricuspid annular systolic motion on DTI.
