毫火针赞刺法与毫火针加灸法治疗急性期中老年带状疱疹平行对照临床研究

目的 比较毫火针赞刺法与毫火针加温和灸法治疗急性期中老年带状疱疹的临床疗效. 方法 将120例患者随机分为毫火针赞刺法组和毫火针加灸法组各60例,在毫火针点刺的基础上两组分别予以拔罐和温和灸治疗.每个疗程治疗2～6次；若未完全好转,可再进行下一个疗程,共观察21天.记录两组患者的止痛时间、结痂时间和治疗次数,检测两组患者治疗前,治疗第2次、第6次、最后1次的疼痛程度,过去24h内最痛程度及皮损面积,并于治疗结束后评价两组患者皮损面积、局部痛及综合疗效. 结果 毫火针赞刺法组止痛时间、结痂时间均早于毫火针加灸法组(P＜0.01),平均治疗次数少于毫火针加灸法组(P＜0.01).两组患者治疗第2次、第6次、最后1次过去24h内最痛程度、皮损面积均较上次治疗时明显减轻(P＜0.01)；两组各时间点过去24h内最痛程度、皮损面积组间比较差异无统计学意义(P＞0.05).皮损面积疗效毫火针加灸法组总有效率67.8％,毫火针赞刺法组为70.7％,两组比较差异无统计学意义(P＞0.05)；局部痛疗效毫火针加灸法组总有效率93.3％,毫火针赞刺法组为100％,两组比较差异有统计学意义(P＜0.05)；综合疗效毫火针加灸法组总有效率94.9％,毫火针赞刺组总有效率98.3％,两组比较差异无统计学意义(P＞0.05).结论 毫火针赞刺法与毫火针加温和灸法治疗带状疱疹均安全有效,毫火针赞刺法相对起效快、镇痛效果好,而毫火针加温和灸法适用于局部疱疹化脓者.

Parallel-Controlled Clinical Study on Assistance Fili-Fire Needling and Fili-Fire Needling Combined with Moxibustion for Acute Herpes Zoster in the Elderly

Objective To compare the therapeutic effects of assistance fili-fire needling(AFFN) and fili-fire needling combined with moxibustion(FFNM) on acute herpes zoster in the elderly.Methods Totally 120 patients were randomized into the AFFN group and FFNM group,with 60 in each.Cupping and moxibustion was added respectively in the AFFN group and FFNM group on the basis of fili-fire needling,2 to 6 times per course of treatment.One more treatment course could be administrated until fully recovery.The observation lasted for 21 days.The analgesia time,crusting time,treatment times,pain degree before and on the second,sixth treatment and the last treatment,the most pain degree and lesion area within the past 24 hours were observed.Results The analgesia time and crusting time in the AFFN group was significantly shorter than that in the FFNM group(P<0.01).The average number of treatment in the AFFN group was less than that in the FFNM group(P<0.01).The degree of most pain and lesion area decreased significantly within 24h after the 2nd,6th and last treatment in both groups(P<0.01),with no significant difference between groups(P>0.05).The total effective rate of lesion area in the FFNM group and AFFN group was 67.8% and 70.7% respectively,with no significant difference between groups(P>0.05).The total effective rate of local pain in the FFNM group and AFFN group was 93.3% and 100% respectively,with significant difference between groups(P<0.05).The comprehensive therapeutic effect of the FFNM group and AFFN group was 94.9% and 98.3% respectively,with no significant difference between groups(P>0.05).Conclusion Both therapies are effective and safe,especially the assistance fili-fire needling with rapid onset and good analgesic effect.The fili-fire needling combined with moxibustion is suitable for topical herpes purulent.
