输尿管狭窄支架置入术的临床应用

目的探讨塑料支架和金属内支架在输尿管狭窄中的应用价值,寻求支架的合理选用。方法对28例输尿管良恶性狭窄患者,采用经皮肾穿刺顺行法或经尿道逆行法插管,行球囊扩张后支架置入。10例置入金属内支架,18例置入塑料支架。结果手术成功率100%,随访2~48个月,患者尿路梗阻解除,临床症状减轻或消失,感染得到控制,肾功能好转。12例恶性狭窄患者,能耐受常规剂量动脉插管化疗。结论输尿管狭窄支架置入术,具有创伤小,操作简单,疗效显著,值得推广。金属内支架通畅性好,在良性输尿管狭窄中,疗效确切,两种支架可酌情选用。

Clinical application of stent placement in treatment of ureteric stricture

Objective To evaluate the application of double-pigtail stent and metal stent in ureteral strictures and to improve the effects of interventional therapeutic efficacy in benign or malignant ureteral strictures. Methods Twenty-eight cases of ureteric stricture were treated partly with direct dilation by percutaneous renal pelvis puncture or retrograde dilation through urethra with stent implantation.Ten cases were treated with metal stent,and 18 cases with double-pigtail stent. Results Success were gained in all cases with strictures relieved,clinical symptoms relieved or disappeared and also infection controlled.All cases were followed up 2 to 48 months after the proceduce. Conclusions Double-pigtail stent and metal stent implantations are safe,available,acceptable in case of benign or malignant ureteric stricture.
