腹腔镜阑尾切除术与传统开腹阑尾切除术的临床对比研究

目的：比较腹腔镜阑尾切除术和开腹阑尾切除术的临床效果。方法：将138例阑尾炎患者按意愿分为接受腹腔镜阑尾切除术（LA组）和开腹阑尾切除术（OA组）,比较两组的手术时间、术后疼痛、下床时间、肛门排气时间及其他并发症的发生率。结果：与OA组相比,LA组手术时间明显延长（P〈0.01）、术后疼痛程度较轻（P〈0.05）、术后下床时间缩短（P〈0.01）;但两组术后肛门排气时间、切口感染发生率比较差异无统计学意义（P〉0.05）。结论：LA是一项微创、安全的手术,具有术后下床早、并发症少等优点。

Comparision of laparoscopic versus open surgery and traditional open appendectomy for appendicitis

Objective：To compare the clinical effect of laparoscopic versus open surgery（LA） and traditional open appendectomy（OA）.Methods：One hundreds and thirty-eight patients were included in this study performed from September 2007 to March 2011 at the traditional Chinese medical hospital of Bazhong.Patients who gave informed consent were divided into either OA or LA groups by their intention and were operated by McBurney′s or laparoscopic technique,respectively.The data of operation time,time to resume normal activity,short term complications were collected and analysed.Results：The average operation time was longer in LA than in OA（P〉0.01）.The post-operative pain showed less pain in LA at day1 post-operative times（P〉0.05）.Time to resume normal activity was shorter in LA than in OA groups,（P〉0.01）.The complications didn′t show a significant difference between two groups（P〉0.05）.Conclusion：Laparoscopic appendectomy is a useful tool in the treatment of appendicitis.Its advantages lie in its minimal invasiveness,its better cosmetic outcome,its lower rate of complications.
