后外侧微创小切口与传统全髋关节置换术的比较

目的总结后外侧微创小切口全髋关节置换术(MIS-THA)的手术经验及探讨近期疗效。方法连续无选择病例采用MIS-THA50例(微创组),同期采用传统全髋关节置换术(THA)50例(传统组),对两组手术时间、出血量和术后血红蛋白、红细胞压积、输血情况,切口长度、并发症、术后疼痛、住院时间、假体位置及术后功能康复进行比较。结果微创组切口长度和住院时间分别为8.5 cm、10.5 d,明显短于传统组的15.2cm、11.8 d(P<0.05);微创组术后早期疼痛明显轻于传统组,手术显性失血明显少于传统组(P<0.05);假体位置两组差异无统计学意义。微创组有2例发生切口近端皮肤擦伤,1例皮下血肿;传统组有1例术后发生深静脉血栓,1例脱位。微创组在术后6周和3个月需要助行器者所占比率明显减少(P<0.05);改良的Harris评分在微创组于术后6周明显高于传统组(P<0.05),但在术后第3个月以后,两组差异无统计学意义(P>0.05)。结论后外侧入路微创全髋关节置换术具有软组织损伤小、手术安全、减轻术后疼痛、缩短住院时间和加快术后康复的优势。
