三维能量多普勒超声成像在颈动脉狭窄的应用价值

摘    要:目的评价三维能量多普勒超声成像在诊断颈动脉狭窄中的临床应用价值。方法对55例颈动脉狭窄患者进行自由臂模式三维能量多普勒超声成像,测定其狭窄程度及狭窄百分比,并与二维彩色多普勒超声和数字减影血管造影结果比较。结果三种方法所测狭窄百分比经统计学检验,三者间差异无显著性,且呈高度正相关(r1=0.9863r2=0.9873r3=0.9843)。三维能量多普勒超声具有较高的诊断准确率达96.3%(53/55),并可直观显示狭窄区的立体形状,包括斑块表面特性、狭窄确切部位、血管腔大小及血流空间走行情况。结论三维能量多普勒超声成像可准确显示狭窄的部位、范围及狭窄程度,对明确诊断及确定治疗方案有重要意义,可作为颈动脉狭窄的首选检查方法。

Abstract:Objective To evaluate the clinical value of three-dimensional ultrasound power doppler imaging(3D-PDI) in patients with carotid artery stenosis.Methods The degree and ratio of carotid artery stenosis were measured by 3D-PDI in 55 patients and the result were compared with Color Doppler Flow imaging(CDFI),digital subtraction angiography(DSA).The free-hand mode of acquiring imaging was employed.Results There was no statistically difference between the stenosis ratio by the three methods above.3D-PDI have higher accurate ratio with 96.3%(53/55).3D-PDI of normal carotid artery with atheromatous plaque showed directly the stereo shape,surface character and exact position of plaque,volume of vessel and spatial distribution of blood flow.Conclusion The range and degree of carotid artery stenosis is more directly finded the on 3D-PDI.It can provide more information for diagnosis and treatment.3D-PDI is an accurate,noninvasive method in diagnosis of carotid artery stenosis.
