个案管理护理模式在精神分裂症住院患者中的应用

目的探讨个案管理护理模式对精神分裂症患者生活质量、自知力和治疗态度的影响。方法 2011年1月至2012年6月将110例住院精神分裂症患者随机分为研究组和对照组,研究组实施个案管理护理模式,对照组采用常规护理模式。分别于患者入组时、出院时、出院后6个月及12个月,用精神分裂症患者生活质量量表(the schizophrenia quality of life scale,SQLS)及自知力与治疗态度问卷(insight and treatment attitudes questionnaires,ITAQ)对两组患者进行测评。结果两组患者出院时及出院后6、12个月,SQLS各分量表评分均低于入组时;出院后6、12个月,SQLS各分量表评分又高于出院时;出院时及出院后6、12个月,研究组SQLS各分量表评分均低于对照组同期(P0.05or P0.01)。对照组出院后6、12个月,ITAQ评分明显低于出院时;而研究组出院后6、12个月,ITAQ评分高于入组时;出院时及出院后6、12个月,研究组ITAQ评分均高于对照组同期(P0.05或P0.01)。结论实施个案管理护理模式使精神分裂症患者从医院至社区都得到科学的管理,提升了患者的认知能力及治疗依从性,提高了患者的生活质量和社会功能。

Application of Individual Case Management Nursing Mode in Inpatient with Schizophrenic

Objective To discuss the influences of individual case management nursing mode on life quality,selfconsciousness and attitude towards treatment of schizophrenics.Methods From 2011 January to 2012 June,110schizophrenics were randomly categorized into control group and experiment group.The experiment group received individual case management nursing mode,while the control group received conventional nursing mode.SQLS and ITAQ were used to evaluated two groups of patients the time of enter into group,the time when discharge,and six and twelve months after discharge.Results SQLS score of two groups of patients at the time when discharge and six and twelve months after discharge were lower than the time of enter into group.SQLS score of two groups of patients six and twelve months after discharge were higher than the time when discharge.The SQLS score in experimental group were lower than control group at six and twelve months after discharge and the time when discharge（P〈0.05 or P〈0.01）.The ITAQ score of six and twelve months after discharge of patients in control group was significantly lower than the time of discharge,while the ITAQ score of six and twelve months after discharge of patients in experimental group was significantly higher than the time of enter into group.The ITAQ score in experimental group were higher than control group at the time of discharge and six and twelve months after discharge（P〈0.05 or P〈0.01）.Conclusion The implementation of individual case management nursing model can provide scientific management to patients from admission in hospital to community,and also improve the patient＇s self-consciousness and treatment compliance,thus to improve patient＇s life quality and social function.
