摘    要:目的:观察早期对早产儿使用重组促红细胞生成素(rhEPO)和相应的护理对脑损伤的防治效果。方法选取2010年1月~2013年9月93例早产儿根据家属意愿分成观察组48例和对照组45例。观察组早期给予rhEPO并实施相应护理;对照组给予常规对症处理和常规护理。对比纠正胎龄40周时的神经检测(NBNA)情况及纠正胎龄1个月、3个月、6个月时脑干诱发电位(ABR)检查异常率。结果纠正胎龄40周时NBNA评分观察组较对照组高,差异有统计学意义(P<0.05);两组纠正胎龄1个月时ABR异常率差别不明显(P>0.05);纠正胎龄3、6个月时ABR异常率观察组均较对照组低,差异有统计学意义(P<0.05)。结论对早产儿早期使用rhEPO并施以相应护理可有效的防治脑损伤的发生及减轻脑损伤的程度。

Clinical observation of recombinant erythropoietin in early prevention and treatment of brain injury in preterm infants

Abstract:Objective To observe the clinical effect of recombinant erythropoietin in early prevention and treatment of brain injury in preterm infants, and summarize the nursing experience.Methods 93 cases of premature infants were divided into observation group with 48 cases and control group with 45 cases according to the parents’ wishes. Observation group was treated with rhEPO and implement the corresponding nursing, while babies in the control group were being given routine symptomatic treatment and routine nursing care. Compared to score of Neonatal behavioral neurological assessment(NBNA) at rectify 40 weeks of gestational age and abnormality rate of Auditory Brainstem Response(ABR) at 1 month, 3 months, 6 months.Results The NBNA score of observation group at 40 weeks corrected gestational age was higher than that of the control group(P〈0.05);The abnormal rate of ABR of the two group was not different from each other at 1 month(P〉0.05), and the abnormal rate of ABR of observation group was lower than that in the control group at 3, 6 months(P〈0.05).Conclusion Early use of rhEPO in premature infants with damage corresponding nursing care can effectively prevent and alleviate the severity of brain injury brain.
