大剂量脉络宁治疗AMI再灌注损伤

摘    要:选择具有溶栓指征的急性心肌梗塞(AMI)109例,施行尿激酶静脉溶栓治疗,冠脉再通80例,分治疗组42例,于溶栓前10 ̄30min给脉络宁40 ̄50ml,并以此剂量每日1次连用7d;设单用溶栓治疗者38例为对照组进行比较。结果:两组有效率分别为97.6%和84.2%,发生严重心衰3例(7.1%)和6例(15.8%),发生再灌注心律失常(RPA)17例(40.5%)和30例(78.9%),两组间差异

Abstract:Choicing 109 AMI patients who adapted the thrombolytic, We treated them with injecting urokinase, Among them there were 80 coronary urtery recirculation. Then we chose 42 patients to be injected MLN 40-50ml for 10-30 minutes befor the treatment and continued to use this dosage once a day. The rest 38 patients were to be observed, Then we compared the two groups, The result showed that 2 patients (3. 8%)and 6 patients(12.2%)died, 3 patients (7. 1%)and 6 patients(15. 8)presented serious heart failure. Besides, 17 patients(41. 5%) and 38 patients (77.6%)presented reinfusion arrhythmia. From these results we conduded that MLN could decrease the oxygen consumption of myoardium, decrease the area of myocardical infarction, decease the release of the serum myocardium enzyme.Furthermere it could increase the ejection function of the left-ventride. Ware, the big dosage MLN can produce the funtion which resist acutemyocardical infarction reinfusion injury.
