冠心病患者脑钠肽与冠状动脉病变程度的相关性研究

摘    要:目的探讨脑钠肽与冠心病患者冠状动脉(冠脉)病变严重程度的相关性。方法入选行冠脉造影检查的冠心病患者173例,所有患者入院后均行冠脉造影检查及血浆脑钠肽(BNP)检查,分析不同冠脉病变支数、冠心病分型、冠脉病变程度(Gensini积分)与患者血清BNP水平之间的关系。结果随着冠脉病变支数增加,患者BNP水平呈显著增加趋势(P0.05)。稳定型心绞痛、不稳定型心绞痛、心肌梗死三组患者BNP水平呈显著增加趋势,三组BNP水平之间差异有统计学意义(P0.05)。BNP100pg/ml的患者Gensini积分显著高于BNP100 pg/ml的患者Gensini积分,分别为(53.7±15.9)分vs.(29.3±20.7)分,差异具有统计学意义(P0.05)。Gensini积分与BNP水平存在显著正相关(r=0.476,P0.05)。结论 BNP水平与冠心病患者冠脉病变严重程度正相关,BNP水平有可能作为冠心病患者冠脉病变严重程度的判断指标。

Correlation between B-type natriuretic peptide and severity of coronary artery lesion in patients with coronary heart disease

Abstract:Objective To discuss the correlation between B-type natriuretic peptide (BNP) and severity of coronary artery lesion in patients with coronary heart disease (CHD). Methods The patients (n=173) were chosen and given examinations of coronary angiography (CAG) and level of plasma BNP. The relationship between number of diseased coronary vessels, CHD types, severity of coronary lesion (Gensini integral) and level of BNP was analyzed. Results With the increase of number of diseased coronary vessels, the level of BNP showed an increasing trend (P〈0.05), and that increased in stable angina group, unstable angina group and myocardial infarction group (P〈0.05). Gensini integral was significantly higher in patients with BNP〈100 pg/mL than that in those with BNP〈100 pg/mL (73.7±35.9 vs.45.3±20.7, P〈0.05). Gensini integral was positively correlated to the level of BNP (r=0.476, P〈0.05). Conclusion The level of BNP is positively correlated to the severity of coronary lesion, which can be taken as an index for judging the severity of coronary lesion in CHD patients.
