加味玉液汤辅助西药治疗2型糖尿病疗效及对血糖波动幅度和胰岛素抵抗的影响

摘    要:目的:观察加味玉液汤辅助西药治疗2型糖尿病(T2DM)疗效及对血糖波动幅度和胰岛素抵抗的影响。方法:选择医院2016年1月~2018年1月收治2型糖尿病患者326例,按随机数表法分为两组。对照组采用常规西药治疗,服用二甲双胍,0.5g/次,1次/天;观察组在对照组基础上服用加味玉液汤,将药材煎煮至600ml,分三次服用,1剂/天;两组患者均治疗2个月。统计两组患者疗效,统计两组患者治疗前后全天血糖标准差(SDBG)、1天内平均血糖波动幅度(MAGE)、最大血糖波动幅度(LAGE),统计两组患者治疗前后FPG、2hPG、HbA1c水平,统计两组患者空腹胰岛素(FINS)水平、胰岛素分泌指数(Homa-β)、胰岛素抵抗指数(Homa-IR)。结果:观察组治疗有效率(96.39%)明显高于对照组(89.38%),差异有统计学意义(P<0.05);治疗后,观察组SDBG、MAGE、LAGE水平均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组FPG、2hPG、HbA1c水平均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组Homa-IR水平低于对照组,FINS、Homa-β水平高于对照组,差异有统计学意义(P<0.05)。结论:加味玉液汤辅助西药治疗T2DM降低血糖效果显著。

Abstract:Objective:To study the efficacy of Modified Yuye Decoction assisted with western medicine on type 2 diabetes mellitus(T2 DM) and its effects on blood glucose fluctuation range and insulin resistance.Methods:326 cases of patients with T2 DM in the hospital from January 2016 to January 2018 were selected and divided into two groups according to the random number table method.The controlled group was given conventional western medicine of metformin for 0.5 g/time and once/d, and the observation group was treated with Modified Yuye Decoction on the basis of the controlled group, and the medicine materials were boiled to 600 ml for three times of taking(1 dose/d).All patients were treated for two months.The efficacy of the two groups was counted.The standard deviation of blood glucose(SDBG), the mean amplitude of glucose excursions(MAGE) within 1 d and largest amplitude of glycemic excursions(LAGE) were counted in the two groups before and after treatment.The levels of FPG, 2 hPG and HbA1 c were measured before and after treatment.The fasting insulin(FINS) level, insulin secretion index(Homa-β) and insulin resistance index(Homa-IR) were calculated in the two groups.Results:The effective rate of treatment in the observation group was significantly higher than that in the controlled group(96.39% vs 89.38%)(P<0.05).After treatment, the levels of SDBG, MAGE and LAGE in the observation group were lower than those in the controlled group(P<0.05).After treatment, the levels of FPG, 2 hPG and HbA1 c in the observation group were lower than those in the controlled group(P<0.05).After treatment, the Homa-IR level in the observation group was lower than that in the controlled group while the levels of FINS and Homa-β were higher than those in the controlled group(P<0.05).Conclusions:Modified Yuye Decoction assisted with western medicine in treating T2 DM has significant effects on lowering blood glucose.
