健脾益气防感汤治疗小儿反复呼吸道感染临床疗效观察

目的:观察健脾益气防感汤治疗小儿反复呼吸道感染(RRTI)的临床疗效。方法:选取2017年1月~2017年12月我院收治RRTI患儿88名,根据随机数字表,分为观察组及对照组各44例,对照组采取化痰、止咳、退热、调节免疫等对症处理,观察组在对照组之上,加服健脾益气防感汤,观察两组用药期间症状缓解时间,治疗2个疗程后,比较两组中医症候积分及临床疗效,检测血清免疫球蛋白[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]水平以及T细胞亚群[T淋巴细胞3(CD3)、诱导性T细胞4(CD4)、抑制性T细胞8(CD8)及CD4/CD8比值]变化。结果:治疗期间,观察组咳嗽咳痰、发热、鼻涕鼻塞、咽喉充血及肺部啰音症状缓解时间均显著短于对照组(P<0.05);治疗后,两组中医症候积分均显著低于治疗前(P<0.05),且观察组低于对照组(P<0.05);观察组临床疗效95.45%,显著高于对照组的77.27%(P<0.05);两组血清IgA、IgG、IgM均显著高于治疗前(P<0.05),且观察组上升幅度大于对照组(P<0.05);两组CD3、CD4、CD4/CD8比值均上调(P<0.05),CD8均下调(P<0.05),且观察组各指标改善程度优于对照组(P<0.05)。结论:健脾益气防感汤用于小儿RRTI中,能更好缓解临床症状,减轻中医症候,提高临床疗效,同时能改善患儿体液免疫及细胞免疫。

Clinical efficacy of Jianpi Yiqi Fanggan decoction on children with recurrent respiratory tract infection

Objective:To observe the clinical efficacy of Jianpi Yiqi Fanggan decoction on children with recurrent respiratory tract infection(RRTI).Methods 88 cases of children patients with RRTI from January 2017 to December 2017 in our hospital were selected and divided into observation group and control group with 44 cases in each group according to the random number table method.Control group was given symptomatic treatment of phlegm resolving,cough relieving,antipyretic,and regulating immunity phlegm,cough,defervescence and immune regulation,and observation group was additionally given Jianpi Yiqi Fanggan decoction on the basis of control group.The symptoms relief time during medication was observed in the two groups.After 2 courses of treatment,the TCM syndromes scores and clinical efficacy were compared between the two groups,and the levels of serum immunoglobulins[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)]and T cell subsets[T lymphocyte 3(CD3),inducer T cell 4(CD4),suppressor T cell 8(CD8),and CD4/CD8ratio]were detected in the two groups.Results During treatment,the symptoms relief times of cough and sputum,fever,nasal congestion,throat congestion,and pulmonary rales in observation group were significantly shorter than those in control group(P<0.05).After treatment,the TCM symptoms scores in the two groups were significantly lower than those before treatment(P<0.05),and the scores in observation group were lower than those in control group(P<0.05).The clinical efficacy in observation group was significantly higher than that in control group(95.45%vs 77.27%)(P<0.05).The levels of serum IgA,IgG and IgM were significant higher than those before treatment(P<0.05),and the increase in observation group was higher than that in control group(P<0.05).The levels of CD3,CD4and CD4/CD8ratio were up-regulated(P<0.05)while the level of CD8was down-regulated(P<0.05),and the improvement of each index in observation group was better than that in control group(P<0.05).ConclusionsJianpi Yiqi Fanggan decoction combined with western medicine for children with RRTI can better relieve clinical symptoms,relieve TCM symptoms,and improve clinical efficacy,and can promote humoral immunity and cellular immunity of children patients.
