血清胃蛋白酶原、胃泌素与Hp相关性食管炎病情程度相关性的评价

目的：探讨血清胃蛋白酶原、胃泌素在幽门螺杆菌（Hp）感染相关性食管炎病情程度中的评估价值。方法：选取105例食管炎患者，其中细菌培养Hp阳性者作为Hp相关性食管炎组，Hp阴性者作为非Hp相关性食管炎组，采用ELISA法检测血清Hp-IgG抗体、PGⅠ、PGⅡ、胃泌素含量和PGR（PGⅠ/ PGⅡ）水平，病情程度按内镜下食管黏膜病变程度分级（A、B、C、D级）。结果：食管炎分级越高，血清Hp-IgG 抗体阳性比例越高，其中A级食管炎患者Hp-IgG 抗体阳性比例明显低于D级食管炎患者，A、D级别比较差异具有统计学意义（P＜0.05），Hp相关性食管炎组PGⅠ、PGⅡ、胃泌素和PGR均明显低于非Hp相关性性食管炎组，两组比较差异均具有统计学意义（P＜0.05），且不同病情程度Hp相关性食管炎组患者PGⅠ、PGⅡ、胃泌素和PGR比较差异均有统计学意义（P＜0.05），其中A级最高，D级最低。结论：食管炎与Hp感染密切相关，血清胃蛋白酶原和胃泌素水平在Hp相关性食管炎患者中明显增高，且对其病情程度评估具有重要价值。

Correlative evaluation of serum pepsinogen and gastrin with severity of Hp-related esophagitis

Objective: To investigate the evaluation value of serum pepsinogen and gastrin in the severity of Helicobacter pylori (Hp)-related esophagitis.Methods: One hundred and ifve cases of patients with esophagitis were selected and divided into an Hp-related esophagitis group (Hp positive) and a none-Hp-related esophagitis group (Hp negative). The contents of serum Hp-IgG antibody, PGⅠ, PGⅡand gastrin and PGR (PGⅠ/PGⅡ) levels were detected by ELISA method and the severity of disease was graded based on endoscopic esophageal mucosa lesion severity (A, B, C, D).Results: The higher the classiifcation of esophagitis, the higher the positive ratio of Hp-IgG serum antibody, the positive ratio of Hp-IgG antibody in patients with grade A esophagitis was signiifcantly lower than grade D esophagitis (P<0.05), PGⅠ, PGⅡ, gastrin and PGR levels were signiifcantly lower in the Hp-related esophagitis group than none-Hp-related esophagitis group(P<0.05), and a comparison of PGⅠ, PGⅡ, gastrin and PGR levels among different severity of Hp-related esophagitis patients showed statistical signiifcance (P<0.05), in which the highest was grade A and the lowest was grade D.Conclusion: Esophagitis is closely related to Hp infection. The levels of serum pepsinogen and gastrin are signiifcantly increased in Hp-related esophagitis, which has an important value in the evaluation of disease severity.
