高血压病并发阵发性心房颤动患者左心房电生理特点

目的探讨高血压病患者和高血压病并发阵发性心房颤动患者左心房电生理特性变化。方法应用二维超声心动图及多普勒、多普勒组织成像（DTI）技术，测定和计算30例正常人、25例高血压病病程≤10年患者、24例高血压病病程＞10年患者、28例高血压病并发阵发性心房颤动患者的左心房电生理指标：电-机械时间（P-A）和电-机械时间离散度（P-Ad）、左、右心房电传导整体差异（DQAT）、体表心电图P波最大时限（Pmax）和P波离散度（Pd）。 结果高血压病并发阵发性心房颤动组三尖瓣环电机械时间（P-A3）较正常对照组、高血压病病程≤10年组、高血压病病程＞10年组明显延长（P〈0.01、P〈005、P〈0.05），高血压病并发阵发性心房颤动组室间隔三尖瓣环电机械时间离散度（P-Ad2）较正常对照组、高血压病病程≤10年组、高血压病病程＞10年组绝对值明显延长（P〈0.05、P〈0.05、P〈0.05）。结论①高血压病并发阵发性心房颤动患者心房电机械时间和电机械时间离散度延长;②某些反映左心房电生理特性的心脏超声指标如P-A3、P-Ad2可以用于预测高血压病患者并发阵发性心房颤动的发生。
