摘    要:目的探讨手部肌腱损伤的超声动态声像图特征,为肌腱病变的超声鉴别诊断提供依据。方法182例临床疑诊手部肌腱损伤患者,超声重点观察屈肌腱或者伸肌腱在被动或者主动运动时,肌腱连续性和肌腱异常回声区的声像图变化等表现。结果根据损伤肌腱不同的动态声像图表现,超声提示肌腱撕裂121例,其中完全撕裂86例,不完全撕裂35例。超声提示肌腱局部肿胀者30例,缝合处粘连31例。1324例得到手术证实,50例通过超声随访以及临床症状的缓解而得到了验证。结论利用超声的动态探测方法学优势,动态声像图可为手部肌腱损伤的鉴别诊断提供重要依据。

Abstract:Objective To evaluate the dynamic ultrasonography of the hand tendon tears in the differentia/diagnosis of tendon diseases. Methods One hundred and eighty-two patients suspected the hand tendon injuries clinically were examined by dynamic ultrasonography. The sonographic presence of tendon continuity and abnormal echogenicity of tendon were observed in tendon movement, especially in the passive or active movement of flexor tendon and extensor tendon. Results According to the sonographic presence for hand tendon injuries, 121 cases were tendon tear (86 complete tear and 35 incomplete tear), 30 cases were local tumefaction in tendon and 31 cases were conglutination in suture. One hundred and thirty-two cases were confirmed by operation and 50 were confirmed by follow-up of ultrasonography and clinica/relief.Conclusion Ultrasonography has its predominance in dynamic examination, which provides important information for the differential diagnosis of tendon injuries of hand.
