摘    要:目的 探讨肢体运动对早产儿骨矿物质含量(骨矿含量)的影响及临床意义。方法 用单光子吸收法(SPA)测定12例(运动组)早产儿运动4周前后桡骨骨矿含量值,同期检测血清钙(Ca)、磷(P)、碱性磷酸酶(AKP)、骨钙素(BGP)及甲状旁腺素(PTH)水平,并与11例(对照组)早产儿进行比较。结果 研究结束时运动组骨线密度(BMC)、骨面密度(BMD)值明显大于对照组水平(P均<0.01)。研究期间运动组BMC、BMD和骨宽度(BW)分别增加30%、15%和11%;对照组BW仅增加6.4%,而BMC、BMD则减少11%、14%。同期发现运动后血清BGP值明显高于运动前水平(P<0.05),对照组4周结束时PTH值明显大于开始时水平(P<0.05)。结论 运动可增加早产儿骨矿化作用,防止其骨质减少,这对预防早产儿代谢性骨病(MBDP)有重要意义。运动后骨矿含量的增加可能与增加骨形成、减少骨吸收有关

Effect of Physical Activity on Bone Mineralization in Premature Infants
Abstract:To explore the effect of physical activity on bone mineralization in preterm infants. Methods Bone mineral analyses of single photon absorptiometry(SPA) and the serum levels of calcium(Ca),phosphate(P),alkaline phosphatase(AKP),parathyroid hormone(PTH) and bone gla protein(BGP) were measured in 12 cases of prematurity(group EX) before and after 4 week physical activity,and were compared with those of 11 cases of prematurity(group C). Results Bone mineral content(BMC) and bone mineral density(BMD) values in group EX were significantly higher than those in group C ( P <0.01).During the 4 week study,group EX infants had 30%,15% and 11% gains in BMC,BMD and bone widty (BW),respectively;group C infants had only a 6.4% gain in BW and 11% and 14% losses from baselinein BMC and BMD.Compared with the baseline,BGP levels in group EX were significantly increased at the end of the study completion( P <0.05) and PTH levels in group C were significantly increased ( P <0.05). Conclusions Physical activity can promote bone mineralization and prevent bone mass loss.It would be an important clinical value to take physical activity to prevent metabolic bone disease of prematurity(MBDP).The gain of bone mineralization by physical activity is probably related to the increase of bone formation and the decrease of bone absorption.
