氨氯地平对不同性别高血压患者心脏结构与功能的影响

摘    要:目的 探讨氨氯地平对不同性别老年高血压患者心脏结构与功能的影响.方法 选择46例高血压患者,分为男性组27例,女性组19例.所有患者使用氨氯地平治疗,比较治疗前及治疗1年后不同性别组心脏结构和功能的变化.结果 治疗后,男性及女性的二尖瓣最大舒张期血流A峰速度较前下降,E峰速度/A速峰度比值(E/A)较前上升(P<0.05),但性别对A峰速度及E/A随时间变化的趋势均无影响(P>0.05);性别对左心室重量指数(LVMI)、左室射血分数(LVEF)、舒张末期左室内径随时间变化的趋势均有影响(P<0.05),其中男性患者治疗后的LVEF呈上升趋势而LVMI呈下降趋势,而女性患者则相反.结论 氨氯地平对心脏结构的影响存在性别差异,其对男性的保护作用可能大于女性.

Abstract:Objective To explore the effects of amlodipine on the cardiac structure and function in elderly hypertensive patients of different gender.Methods A total of 46 hypertensive patients were enrolled and were divided into male group(n=27) and female group(n=19).All subjects were treated with amlodipine,and the cardiac structure and function were compared in different gender groups before treatment and after one year of treatment.Results After treatment,the maximal diastolic mitral peak velocity of the A wave decreased in the male and female patients(P<0.05),And the ratio of peak velocity of the E wave to peak velocity of the A wave(E/A) increased,but gender had no effect on the tendency that peak velocity of the A wave or E/A chanced over time(P>0.05).Gender had effects on the tendency that left ventricular mass index(LVMI),left ventricular ejection fraction(LVEF) and left ventricular end-diastolic diameter(LVEDd) changed over time(P<0.05).After treatment,LVEF increased and LVMI decreased in the male patients,while LVEF decreased and LVMI increased in the female patients.Conclusion There is gender difference in the influence of amlodipine on cardiac structure.Amlodipine might have more protective effect on the male than the female.
