成人和儿童急性白血病常见融合基因表达的比较

摘    要:目的 比较成人和儿童急性白血病常见融合基因表达的差异.方法 采用多重巢式PCR技术检测159例成人和120例儿童初治急性白血病患者的常见融合基因,比较成人及儿童各融合基因的阳性率.结果 儿童急性淋巴细胞白血病(ALL)患者白血病融合基因总阳性率为51.11%(46/90),高于成人ALL患者的25.53%(24/94)(P<0.05);其中儿童TEL/AML1及TLS/ERG融合基因阳性率高于成人(P>0.05).儿童急性髓系细胞白血病(AML)患者白血病融合基因总阳性率为53.33%(16/30),高于成人AML患者的26.15%(17/65)(P<0.05);其中儿童AML1/ETO融合基因阳性率高于成人(P>0.05).结论 成人及儿童急性白血病融合基因的表达存在一定差异,临床上应有针对性地进行相关融合基因的检测和判读,以提高诊疗质量.

Abstract:Objective To compare the expressions of common fusion genes between adults and children with acute leukemia.Methods The expressions of common fusion genes were detected in 279 patients with initial acute leukemia,including 159 adults and 120 children,by multiplex nested PCR.Then the positive rates of all fusion genes were compared between the adults and the children.Results The overall positive rate of leukemia fusion genes in the children with acute lymphoblastic leukemia(ALL) was higher than that in the adults with ALL[51.11%(46/90) vs.25.53%(24/94);P<0.05].The positive rates of TEL/acute myeloid leukemia(AML)1 and TLS/ERG fusion gene in the children were higher than those in the adults(P>0.05).The overall positive rate of leukemia fusion genes in the children with AML was higher than that in the adults with AML[53.33%(16/30) vs.26.15%(17/65);P<0.05],and the positive rate of AML1/ETO fusion gene in the children was higher than that in the adults(P>0.05).Conclusion There are some differences in the expressions of fusion genes between adults and children with acute leukemia,and the corresponding detection and interpretation of related fusion genes should be carried out to improve the quality of diagnosis and treatment.
