NT-proBNP诊断心衰患者的临床诊断意义探讨

目的 研究NT-proBNP水平对心力衰竭的诊断意义.方法 对2009年至2011年间我院收治的80例心力衰竭患者设为实验组,同期来院体检的健康志愿者40例作为对照组,对两组受试者均检测血NT-proBNP水平及测定左室射血分数(LVEF).结果 对照组NT-proBNP水平为( 56.4±34.7) pg/ml,实验组NYHA Ⅰ级为(238.6±115.7) gp/ml,Ⅱ级为(446.3±253.8) pg/ml,Ⅲ级(1045.6±462.4) pg/ml,Ⅳ级(1803.2±977.6)pg/ml,平均为(1047.5±842.3)pg/ml,显著高于对照组(P＜0.05).实验组NYHA Ⅰ级LVEF为(56.3±11.4)％,Ⅱ级(47.5±10.7)％,Ⅲ级(41.3±9.4)％,Ⅳ级(37.6±8.3)％,NT-proBNP水平与分级成正相关(P＜0.05),与LVEF成负相关(P＜0.05).结论 NT-proBNP水平与心脏功能成负相关,可以有效反映心力衰竭程度,对心衰诊断和心功能评价有重要意义.

Discuss on clinical value of NT-proBNP on diagnosis for heart failure patients

ObJective To research the diagnositic value of NT-proBNP on heat failure （HF）. Methods 80 patients of HF from 2009 to 2011 were selected as experimental group and 40 healthy volunteers were chosen as control group. NT-proBNP level and left ventricular ejection fraction （LVEF） were detected. Results the NT-proBNP of experimental group NYHA I （238.6 ~ 115.7） gp/ml, ]] （446.3 ~ 253.8） pg/ml, ~1 （1045.6 ~ 462.4） pg/m, ]Y（1803.2 ~ 977.6） pg/ml] was significantly higher than that of control group （56.4 ~ 34.7）pg/ml]. The LVEF of experimental group NYHA I was （56.3 ~ 11.4）%, NYHA 2 11 （47.5 ~ 10.7）%, NYHA ]I （41.3 ~ 9.4）%, and NYHA IV （37.6 ~ 8.3）%. The NT- proBNP level was in positive correlation with NYHA grade（r--0.9553, P〈0.05） and in negative correlation with LVEF（r=-0.8968, P〈0.05）. Conclusion The NT-proBNP can reflect the injury of heart function and is negatively correlated with heart function, showing great value for diagnosis of HF.
