急性颈髓损伤合并抗利尿激素分泌异常综合征的影响因素

目的 探讨急性颈髓损伤时，颈髓损伤的发病率及颈髓损伤严重程度、损伤平面、年龄、感染及颅脑损伤与抗利尿激素分泌异常综合征（SIADH）之间的关系。方法 对37例颈髓损伤患者按年龄、损伤程度、损伤平面、合并感染及颅脑损伤情况分组，而后进行统计学分析。结果 本组共19例合并SIADH，发病率为51.35%。年龄、颈髓损伤的程度，损伤平面与SIADH的发病率无关；与颈髓损伤的程度有明显关系；合并感染特别是肺炎SIADH的发病率明显增高。结论 急性颈髓损伤时SIADH的发病率与颈髓损伤的程度和感染有明显关系。与年龄、损伤平面无关。颅脑损伤可能是SIADH的重要诱因。

Acute cervical spinal cord injury associated with syndrome of abnormality of antidiuratic hormone secretion an analysis of the influencing factors

Objective To explore the relationships between the syndrome of abnormal antidiuratic hormone secretion(SIADH) with the incidence,degree of seriousness of cervical spinal cord injury,traumatic level,age,infection and cranio cerebral injury Methods 37 cases of cervical spinal cord injury were classified by their age,degree of seriousness of injury,combined infection and cranial cerebral injury,and then made a statistical analysis.Results In this series of 37 cases of cervical spinal cord injuries,the incidence of combined SIADH was 51 35%(19/37),of which,the incidence of SIADH was not related to their age and the levels of trauma of cervical spinal cord,however,it was clearly related to the degree of seriousness of cervical spinal cord injury,there was a sharp increase in the incidence of SIADH resulted from combined infection,especially pneumonia.Conclusion The incidence of SIADH is clearly related to the degree of seriousness of cervical spinal cord injury and the infection but not to their age,traumatic plane in acute cervical spinal cord injury.And cranio cerebral injury may be the inducing facfor of SIADH.
