改良小梁切除术160例临床分析

目的探讨小梁切除术的改良方法及疗效。方法对160例（165只眼）原发性闭角型青光眼采用1／3厚度的浅层巩膜瓣、术中前房重建及应用外置可调节缝线的改良小梁切除术，观察术后眼压、滤过泡、视力及术后并发症情况，随访6个月～10年，平均6年2个月。结果165只眼中，158只眼（95．76％）术后眼压控制优良，功能性滤过泡形成157只眼（95．15％），发生Ⅰ、Ⅱ级浅前房11只眼（6．67％），无其他严重并发症发生。结论该改良技术治疗青光眼有助于术后早期前房及滤过泡形成，能主动调整滤过功能，减少并发症发生，提高手术效果，且操作简单易行，便于临床推广应用。

Clinical Analysis of Improved Trabeculectomy in 160 Cases

Objective To investigate the improved trabeculectomy methods and observe it＇ s effect. Methods 160 patients （165 eyes） with primary angle-closure glaucoma were treated by modified trabeculectomy. The intraocular pressure （lOP）, the filtering blebs,the visual acuity and the complications after operation were measured. Followed-up time was from 6 months to 10 years, average was 6 years and 2 months. Results Among 165 eyes, 158 eyes （95, 76% ） were well controlled in IOP after operation, the functional filtering blebs occurred in 157 eyes （95.15 % ）, 11 eyes （6.67%） occurred Ⅰ/Ⅱ shallow anterior chamber, no other serious complications occurred. Conclusions The improved technology for the treatment of glaucoma contributes to the formation of the early postoperative anterior chamber and the functional filtering blebs. It can take the initiative to adjust filtering function, reduce complications and improve surgical results, and it is easy, so it is advantageous for the clinical application.
