E-钙粘素及p53表达与壶腹周围癌的分化、浸润及转移的关系

目的：探讨E－钙粘素（E－Cd）及p53在壶腹周围部中的表达，及与壶腹周围癌的分化、浸润及转移的关系。方法：采用免疫组化S－P法检测44例壶腹周围癌组织和11例炎性壶腹部粘膜组织E－钙粘素及p53的表达，结果：（1）77.3%（34／44）壶腹周围癌E－Cd异常表达，86.7%的中、低分化肿瘤（13／15）E－Cd表达异常，浸润范围广泛的肿瘤有81.8%（27／33）E－Cd表达异常，有转移的肿瘤中81.25%（13／16）E－Cd表达异常。（2）38.6%（17／44）壶腹周围癌p53阳性表达。（3）E－Cd及p53的联合检测可能有助于壶腹周围癌的预后推测。

Relationship between Expressions of E-cadherin and p53 and Differentiation,Invasion and Metastasis of Peri-ampullar Carcinoma

Objective: To study the relationship between expressions of E-cadherin (E-Cd) and p53 and differentiation, infiltration and metastasis of peri-ampullar carcinoma. Methods: Immunohistochemical technique (S-P method) was applied to detect the expression of E-Cd in 44 cases of peri-ampullar carcinoma and 11 cases of inflammatory ampullar mucosa tissues. Results: (1) The abnormal rate of E-Cd expression in 44 cases of peri-ampullar carcinoma was 77.3 % (34/44); that of tumors whose differentiation were middle or low was 86.7%(13/15); that of tumors which had a wide range of infiltration was 81.8% (27/33); that of tumors which had a metastasis was 81.25% (13/16). (2)The positive rate of p53 expression in 44 cases of peri-ampullar carcinoma was 38.6% (17/44). (3)The abnormal rate of E-Cd expression in cases that had positive expression of p53 76.5% , 69.2% of which had a wide range of infiltration. Conclusion: (1) The abnormal E-Cd expression is one of malignant phenotypes of peri-ampullar carcinoma and is related to differentiation, infiltration and metastasis. Its rate is high. (2) The expression of p53 reflects the invasive ability of tumors in progressive stage to a certain extent. (3) The combined examination of E-Cd and p53 may be conducive to guess the prognosis of peri-ampullar carcino-
