肌钙蛋白T和B型脑钠肽对心力衰竭患者预后评价的临床意义分析

目的 探讨肌钙蛋白T和B型脑钠肽对心力衰竭患者预后评价的临床意义.方法 所选的100例心力衰竭患者根据心功能评定标准进行心功能分级,分为Ⅱ级组、Ⅲ级组、Ⅳ级组,同时,选择同期到本院体检的健康者60例,作为对照组,测定所选对象的肌钙蛋白T和B型脑钠肽水平.结果 Ⅱ级、Ⅲ级、Ⅳ级组的肌钙蛋白T和B型脑钠肽水平均高于对照组（P＜0.05）,左室射血分数均低于对照组（P＜0.05）;Ⅳ级组肌钙蛋白T和B型脑钠肽水高于Ⅲ级组,左心室射血分数低于Ⅲ级组（P＜0.05）;Ⅲ级组肌钙蛋白T和B型脑钠肽水平高于Ⅱ级组,左心室射血分数低于Ⅱ级组,（P＜0.05）.结论 肌钙蛋白T和B型脑钠肽水平的改变对了解心力衰竭患者的病情严重程度、评估患者预后具有重要意义.

Clinical significance analysis of cardiac troponin T and type B natriuretic peptide for evaluating prognosis of patients with heart failure

Objective To explore the clinical significance of cardiac troponin T and type B natriuretic peptide for eval- uating prognosis of patients with heart failure. Methods 100 cases with heart failure were selected,and they were divid- ed into the grade group,the grade ll group,the grade IV group according to the cardiac function evaluation standard. 60 healthy persons were selected as the control group over the same period.Cardiac troponin T and type B natriuretic peptide level of all subjects were measured. Results The level of cardiac troponin T and type B natriuretic peptide in grade 11, II and 1V group were higher than those in the control group （P〈0.05）,left ventricular ejection fraction in the grade II ,11 and IV group were lower than that in the control group （P〈0.05）;the level of cardiac troponin T and type B natriuretic peptide in the grade 1W group were higher than those in the grade Ill group （P〈0.05）,left ventricular ejection fraction in the grade 1V group were lower than that in the grade llI group （P〈0.05）;the level of cardiac troponin T and type B natriuretic peptide in the grade group were higher than those in the grade II group （P〈0.05）,left ventricular ejection fraction in the grade m group were lower than that in the grade II group （P〈0.05）. Conclusion The level change of troponin T and type B natriuretic peptide has importart significance to understand the severity degree of pa- tients with heart failure and evaluate the prognosis of patients.
