Application of Hybrid Renal Replacement Therapy in Patients With Severe Acute Pancreatitis

Objective：To investigate the therapeutic effects of hybrid renal replacement therapy in patients with severe acute pancreatitis（SAP）.Methods：Thirteen patients treated with hybrid renal replacement therapy from February 2007 to April 2010 were involved in the study,all patients were established vascular access by central venous catheter,heparin or low molecular heparin was used as anticoagulant,and the treatment time was 8~12 hours.Biochemical indexes,APACHE II score and clinical outcome were observed before and after HRRT.Results：Levels of serum creatinine,serum glutamic-oxalacetic transaminease,serum glutamic-pyruvic transaminease,blood amylase,urine amylase were significantly lower than those before HRRT（P0.05）;the APACHE Ⅱ score were 8.2±2.5,6.4±1.6 and 4.1±1.1 at the 1 day,2 days and 7 days after HRRT respectively,which were lower than those before HRRT（16.3±4.6）（P0.01）.One patient died at the first HRRT,another patient gave up continuous treatment after the second HRRT,the rest eleven patients were cured or improved when discharged,the cure and improvement rate was 92.3%,and mortality was 7.7%.Conclusions：HRRT may be an effective supportive treatment for SAP,it can reduce mortality and increase the cure and improvement rate.
