眼球穿通伤所致白内障IOL植入术后继发性近视

目的：分析眼球穿通伤所致白内障ＩＯＬ植入术后继发性近视发生的情况．方法：观察４ 例眼球穿通伤所致白内障ＩＯＬ 植入术后发生继发性近视的原发性损伤、治疗经过、继发性近视的发生／发展及矫正结果．结果：受伤年龄１１岁～３１岁，受伤前均系正视眼，白内障摘除及ＩＯＬ 植入术都在受伤后２月内施行，ＩＯＬ屈光度合适，继发性近视的形成／发展于术后３月以上甚至３年或者更长的时问．结论：眼球穿通伤所致白内障ＩＯＬ 植入术后继发性近视的发生可能与眼穿通伤造成伤眼角膜散先、角膜疤痕、晶体混浊、玻璃体混浊等多种形觉剥夺因素有关．强调对眼外伤尤其严重性眼外伤者长期随访的重要性，及时去除各种形觉剥夺因素，以恢复和保持伤眼的有用视力．

Secondary Myopia after IOL Implantation for Cataract Caused by the Ocular Penetrating Injury

Objective: To analyze the occurrence of the secondary myopia after IOL implantation for cataract caused by the ocular penetrating injury. Methods: The primary injury treatment procedure, occurrence, development of secondary myopia and the results of their correction were observed in 4 cases with the secondary myopia after IOL implantation for cataract by the ocular penetrating injury. Results: All cases were emmetropia before injury and the damaged ages ranged from 11 to 31. The cataract extraction with IOL implantation was performed with 2 months after the ocular penetrating injury. The diopter of IOL was suitable for the injured eyes. The formation/development of the secondary myopia occurred more than 3 months even to 3 years or longer after surgery. Conclusion: The occurrence of the secondary myopia after IOL implantation for cataract caused by the ocular penetrating injury probably related with several factors of pattern deprivation, such as corneal astigmatism, corneal scar, opacities of lens and vitreous caused by ocular injury. It should be emphasized the importance of making a long-term follow-up to the patients with ocular trauma, especially to the severe cases and to remove varied factors of pattern deprivation in time, in order to recover and maintain the useful vision of the injury eyes.
