经导管对称型和偏心型Amplatzer封堵器治疗膜周部室间隔缺损对心脏电生理影响的Meta分析

摘    要:目的:评价对称性和偏心型Amplatzer封堵器治疗膜周部室间隔缺损的有效性和安全性。方法:计算机检索中、外文数据库,纳入相关对照研究,对文献进行质量评价后采用Cochrane系统评价软件RevMan5.0.0进行Meta分析。结果:4篇文献纳入分析,均为单中心非随机对照研究。Meta分析结果显示:对称型封堵器安放成功率优、术后心律失常发生率、术后严重心率失常、术后Ⅲ°房室传导阻滞发生率、术后完全性左束枝传导阻滞发生率均优于偏心型,但均不具有统计学差异。结论:对称型和偏心型封堵器对心肌电生理的影响上与尚无明显差异,暂不能说明偏心型不宜使用。但其相对危险度较高,使用时应慎重。

Abstract:Objective: To evaluate the efficacy and safety of symmetric and asymmetric Amplatzer transcatheter closures for perimembranous ventricular septal defects. Methods: Chinese and foreign databases were searched for controlled trials. The quality of included controlled trials was critically assessed. The Cochrane Collaboration's RevMan 5.0. 0 software was used for data analysis. Results: 4 single center research non-RCTs were involved to analysis. Meta analysis showed: the procedure success rate, the occur rate of cardiac arrhythmia, the occur rate of severe cardiac arrhythmia, the occur rate of Ⅲ°atrioventricular block, and the occur rate of complete left bundle branch block with symmetric device are better than asymmetric device, but there were no significant differenees in all comparisons. Conclusion: There is no significant difference in the impact of eleetrophysiology between symmetric Amplatzer and asymmetric Arnplatzer, and this doesn't show than the asymmetric device is not suitable for clinic treatmeat. However, more care should be took when using asymmetric Amplatzer because of it has a higher risk ratio.
