早期渐进性功能锻炼对乳腺癌术后患肢淋巴水肿干预效果的meta分析

目的：明确早期渐进性功能锻炼对乳腺癌术后患肢淋巴水肿的干预效果。方法计算机检索PubMed、Web of Science、LISTA、Library of Congress、Acad Search Prem、Cochrane、中国生物医学文献数据库、万方数据库、中国知网数据库和维普数据库。检索时间为2001年1月1日~2015年12月31日。由2名研究者独立筛选文献、评价文献质量和提取数据。采用Revman5.3软件做meta分析。结果共纳入9项随机对照试验研究,按随访1个月、3个月、6个月及以上分组,分别做meta分析。结果显示,干预组患者术后1个月( Z=5.83, P<0.00001)、3个月(Z=3.73,P=0.0002)、6个月及以上(Z=3.10,P=0.002)患肢淋巴水肿发生率均低于对照组,差异有统计学意义( P<0.05)。结论早期渐进性功能锻炼能够有效改善乳腺癌术后患肢淋巴水肿,并具有较好的长期效果。

Meta-analysis of effects of early progressive functional exercises on upper limb lymphedema after breast cancer surgery

Objective To identify the effects of early progressive functional exercises on upper limb lymphed-ema after breast cancer surgery. Methods Databases of PubMed, Web of Science, LISTA, Library of Congress, Acad Search Prem, Cochrane, CBM, Wanfang Data, CNKI and VIP were retrieved from 1 January 2001 to 31 December 2015. The selection of literatures, quality evaluation and data extraction were completed by two independent researchers. Revman5. 3 software was employed to perform meta-analysis. Results A total of 9 randomized controlled tri-als were included and were divided into different groups according to the corresponding follow-up time, namely 1 month, 3 months, and 6 months and above. Study results showed that the incidence of suffering upper limb lymphedema of patients in intervention group 1 month ( Z=5. 83 , P<0. 00001 ) , 3 months ( Z=3. 73 , P=0. 0002 ) , and 6 months and above ( Z=3. 10 , P=0. 002 ) after surgery was all lower than that of the patients of control group and the differences were statistically significant ( P<0. 05 ) . Conclusion The early progressive functional exercises have positive effects on improving upper limb lymphedema after breast cancer surgery, and it's long-term effects were also satisfied.
