宫腔镜下子宫内膜电切治疗多发性子宫内膜息肉的效果观察

目的：探究宫腔镜下子宫内膜电切对多发性子宫内膜息肉患者激素水平及免疫功能的影响。方法选取陕西省康复医院自2014年1月至2016年1月收治的90例多发性子宫内膜息肉患者，按照就诊时间顺序分为对照组与观察组，每组各45例，对照组给予宫腔镜下子宫内膜息肉刮宫术，观察组给予宫腔镜下子宫内膜息肉电切术，对比两组患者围术期指标、激素水平及免疫功能的变化。结果观察组与对照组相比随访6个月复发率及异常出血率均较低，，但观察组显著低于对照组，差异具有统计学意义（χ2值分别为4．53、5．02，均P＜0．05）。两组患者治疗后6个月与治疗前相比FSH降低，LH降低，E2降低，观察组较对照组相比上述指标改善更加显著，差异具有统计学意义（t值分别为4．12、4．26、4．53，均P＜0．05）。两组患者治疗后与治疗前相比IgA、IgG水平升高，观察组与对照组相比上述指标改善更加显著，差异具有统计学意义（ t值分别为3．86、4．20，均P＜0．05）。结论宫腔镜下子宫内膜电切术治疗多发性子宫内膜息肉的疗效显著，可有效改善激素水平及免疫功能，复发率及异常出血率较低，安全性更高。

Effect of endometria electrocision under hysteroscopy on multiple endometrial polyps

Objective To explore and analyze the impact of endometria electrocision under hysteroscopy on hormone levels and immunologic function of patients with multiple endometrial polyps . Methods Ninety cases with multiple endometrial polyps treated in Shaanxi Rehabilitation Hospital from January 2014 to January 2016 were selected.All the patients were divided into control group and observation group according to registration order with 45 cases in each group .Endometrial polyps curettage under hysteroscopy was performed on the control group , while the observation group was treated with endometria polyp electrocision under hysteroscopy .Comparison was made in perioperative indicators , hormone levels and the changes of immune function between two groups .Results The recurrence rate and abnormal bleeding rate of two groups during 6-months follow-up visit were low , but they were significantly lower in the observation group than in the control group (χ2 value was 4.53 and 5.02, respectively, both P<0.05).FSH, LH and E2 of two groups 6 months after treatment decreased compared to those before treatment , and the improvements of the observation group were more remarkable than the control group with statistically significant difference (t value was 4.12, 4.26 and 4.53, respectively, all P<0.05).IgA and IgG of two groups after treatment increased compared to those before treatment , and the improvements of the observation group were more remarkable than the control group with statistically significant difference (t value was 3.86 and 4.20, respectively, both P<0.05).Conclusion Endometria electrocision under hysteroscopy for multiple endometrial polyps can achieve remarkable curative effect and improve hormone levels and immunologic function effectively .The recurrence rate and abnormal bleeding rate are relatively low , and the security is higher .
