负性情绪对晚期非小细胞肺癌患者姑息治疗中生活质量的影响

目的 探讨负性情绪对晚期非小细胞肺癌患者姑息治疗中生活质量的影响.方法 选取2013年1月至2015年2月在我科确诊的非小细胞肺癌患者120例作为观察组,同期健康查体者100名作为对照组,分别记录焦虑自评量表(SAS)、抑郁自评量表(SDS)以及生活质量核心问卷量表的分值,并分析比较负性情绪与生活质量的相关性.结果 对照组的SDS、SAS评分分别为(27.53±9.42)分和(31.29±7.50)分,而观察组分别为(38.34±8.63)分和(37.32±7.59)分,2组比较差异有统计学意义(t值分别为8.87、5.90,P值均＜0.05).观察组的整体质量评分为(60.52±12.39)分,处于较差的水平.观察组的负性情绪与整体质量、功能总分呈负相关(r=-0.66～-0.29,P值均＜0.05),而与症状评分呈正相关(r=0.33～0.60,P值均＜0.05).多因素Logistic分析,抑郁(β＝1.730,OR =5.642)、焦虑(β=2.150,OR＝8.588)等负性情绪严重影响观察组的生活质量.结论 负性情绪严重降低晚期非小细胞肺癌患者姑息治疗中的生活质量,及时发现并干预患者的抑郁、焦虑状态对提高患者生活质量尤为重要.

Effect of negative emotions on quality of life of patients with advanced non-small cell lung cancer in palliative treatment

Objective To explore the influence of negative emotions of patients with non-small cell lung cancer on quality of life in palliative treatment.Methods 120 patients with non-small cell lung cancer diagnosed in our hospital from January 2013 to February 2015 were selected as the observation group.At the same time,100 cases of healthy persons were selected as control group.The anxiety self rating scale (SAS),depression self rating scale (SDS) and the questionnaire of life quality core scale score were recorded.The correlation between negative emotions and quality of life was analyzed.Results The scores of SDS and SAS in control group were 27.53± 9.42 and 31.29±-7.50,while they were 38.34±8.63 and 37.32 ± 7.59 in observation group respectively,there was statistical significance between the two groups (t =8.87,5.90,respectively,all P ＜0.05).The overall quality score in observation group was 60.52 ±12.39,which was in a poor level.The negative emotions of observation group were negatively correlated with the total quality and function score (r =-0.66-0.29,all P ＜0.05),but were positively correlated with symptom score (r ＝0.33-0.60,all P ＜0.05).Logistic multivariate analysis showed that depression (β=1.730,OR =5.642),anxiety (β =2.150,OR =8.588) and other negative emotions seriously affected the quality of life of the observation group.Conclusions Negative emotions seriously reduce the quality of life in the palliative treatment of patients with advanced non-small cell lung cancer,the timely discovery and intervention on depression,anxiety status of the patients is very important to improve the life quality of patients.
