老年髋部骨折患者围术期消化系统优化管理及优质护理干预研究

目的：探讨优化管理结合优质护理开展减少高龄髋部骨折患者围术期消化系统并发症的疗效。方法选择2013年8月至2015年6月因髋部骨折住院手术治疗的65岁以上老年患者156例，随机分为观察组和对照组各78例，观察组采用优质护理结合消化系统优化管理，对照组采用常规护理，观察所有患者围术期发生消化系统并发症情况。结果观察组：腹胀2例，便秘3例，呕吐2例；对照组：上消化道出血2例，腹胀10例，便秘16例，腹泻2例，呕吐3例。所有患者发生消化系统并发症后均报告医生后对症处理，对照组死亡1例，其余所有患者出院后骨折均骨性愈合，日常生活大部分能自理。结论对于老年髋部骨折患者，加强消化系统的优化管理结合优质护理的开展可以减少围术期消化系统并发症，改善患者预后，促进患者术后快速康复。

Research on the optimized management for digestive system and high quality nur-sing intervention during perioperative period applied to elderly patients with hip fracture

Objective To explore whether the optimized management combined with high quality nursing can reduce the digestive system complications of elderly patients with hip fracture. Methods A total of 156 cases of elderly patients with hip fracture who were more than 65 years and received operation in our hospital from August 2013 to June 2015 were selected and randomly divided into an observation group and a control group, 78 cases in each group. The observation group adopted high quality nursing combined with digestive system optimized management, while the control group received conventional nursing. The complications of the patients during the perioperative period were compared. Results There were 2 cases of abdominal deistention, 3 cases of constipation and 2 cases of emesis in the observation group. And there were 2 cases of stress upper gastrointestinal bleeding, 10 cases of abdominal deistention, 16 cases of constipation, 2 cases of diarrhea and 3 cases of emesis in the control group. All the patients with digestive system com-plications received proper treatment after being reported to the doctor. There was 1 cases of death in the observation group, while all the other patients all recover after discharge and most of the daily life can be done on their own. Con-clusions The strengthened digestive system optimized management combined with high quality nursing for the elderly patients with hip fracture can reduce the digestive systemˊs complications during the perioperative period and can improve the patientsˊprognosis, and promote the patientsˊrehabilitation after operation.
