MR胰胆管成像诊断肝外恶性胆道梗阻

目的 评价磁共振胆胰管造影（ＭＲＣＰ）对肝外恶性胆管疾病诊断价值。方法 对１５例已证实胆道恶性梗阻患者进行ＭＲＣＰ检查。ＭＲＣＰ采用二维或三维快速自旋回波（ＦＳＥ）序列，结合呼吸门控、指肪抑制技术，采集原始图像后以最大信号强度投影（ＭＩＰ）处理。结果 １５例检查均成功。ＭＲＣＰ对肝外恶性胆道梗阻程度的判断和定位准确率为１００％，定性诊断准确率为８０％。结论 ＭＲＣＯ为非侵袭性技术，恶性胆道梗阻能精

MR cholangiopancreatography in diagnosis of malignant extrahepatic biliary obstruction

Objective To assess the value of MR cholangiopancreatography(MRCP)in the diagnosis of extrahe- patic pancreatobiliary malignancy.methods MRCP was performed in 15 patients with malignant biliary obstruction proved surgically.2D or 3D FSE Sequence,respiratory gating and fat suppression techniques were used.Results MRCP examination succeeded in all 15 patients.The accuracy of MRCP for detecting the degree and level of extrahepatic malignant bile duct obstruction was 100%,and the accuracy for evaluating the causes of obstruction was 80%.Conclusion MRCP is a noninvasive technique with excellent accuracy to define the level of bile duct obstruction.Combining with the source images and routing MRI scanning for analysis,the specificity for the diagnosis of extrahepatic malingant biliary obstruction is also rather high.
