机械通气治疗在急性重度有机磷中毒合并呼吸衰竭患者中的应用

目的：探讨急性重度有机磷中毒合并呼吸衰竭的机械通气效果。方法：对66例急性重度有机磷中毒合并呼吸衰竭患者给予急性气管插管、机械通气,选用SIMV＋PSV模式,根据血气分析结果调整参数,并同时予清除毒物,阿托品、氯磷定等解毒药物应用及对症支持治疗等。结果：3例患者出现呼吸心跳骤停,死于循环功能衰竭,3例放弃治疗,其余60例治愈（90.9%）,机械通气时间18~145h,平均46h,住院6~20d（平均9d）,5例并发呼吸机相关性肺炎。结论：机械通气是抢救急性重度有机磷中毒的有力措施,对于提高该类患者的救治成功率有着非常重要的意义。

The use of mechanical ventilation of severe organophosphorus insecticides poisoning patients

Objective：To observe the effect of mechanical ventilation in the severe organophosphorus insecticides poisoning.Methods：66 cases of severe organophosphorus insecticides poisoned patients,who intubed and mechanical ventilated emergency,the ventilation mode was SIMV＋PSV,we adjust the parameter on the basis of blood gas analysis,at same time,clear the poison,used drugs like atropine and Pyraloxime Chloride.Results：3 patients had cardiac arrest and died of the circulation failure,3 patients gave up the therapy,60 patients were cured（90.9%）.The ventilation time were 18-145 hours,average 46 h,hospital stays 6-20 days（average 9 days）,5 patients had VAP.Conclusion：The treatment of mechanical ventilation is very important to the severe organophosphorus insecticides poisoning patients and it can improve the cure rate.
