下呼吸道感染铜绿假单胞菌的耐药性研究

摘    要:目的:探讨医院下呼吸道感染患者对铜绿假单胞菌的耐药情况变化,以指导临床合理使用抗生素。方法:选取本院呼吸内科2007年8月~2009年10月收治的600例下呼吸道感染患者为研究对象,取患者的痰液进行标本培养,对分离出的63例铜绿假单胞菌进行药敏试验,并对结果进行统计学分析。结果:在600例下呼吸道感染患者中分离出铜绿假单胞菌63株,总分离率为10.5%,在6种抗生素中,亚胺培南的耐药性最低,耐药率为15.87%,其次为阿米卡星,耐药率为30.16%。铜绿假单胞菌对两药耐药率比较,差异有统计学意义(P〈0.05)。结论:铜绿假单胞菌目前仍是医院病原菌感染的主要致病菌之一,临床中要加强耐药性监测,合理使用抗生素。

Abstract:Objective: To investigate the change of drug resistance of Pseudomonas aeruginosa in patients with nosocomial lower respiratory tract infection and guide the rational use of antibiotics. Methods: 600 patients with lower respiratory tract infection in the department of respiratory medicine from March 2007 to May 2009 were selcted as study object. Bacterial culture was carried out in the sputum specimens of all patients. Pseudomonas aeruginosa was isolated from 63 cases, and durg sensitive test was carried out in them. The results were statistically analyzed. Results: 63 strains of Pseudomonas aeruginosa were isolated from 600 patients with lower respiratory tract infection. The total rate of isolation was 10.5%. In 6 kinds of antibiotics, the drug resistance of imipenem was lowest, and the rate of resistance was 15.87%. Amikacin followed imipenem in drug resistance, and the rate of resistance was 30.16%. There was significant difference in 2 kinds of antibiotics’durg resistance(P0.05). Conclusion: Pseudomonas aeruginosa is still a major iatrogenic pathogenic bateria. Clinical monitoring must be strengthened in oder to guide the rational use of antibiotics.
