鼻内镜治疗鼻腔鼻窦内翻性乳头状瘤31例临床分析

目的探讨鼻内镜下切除鼻腔鼻窦内翻性乳头状瘤的方法及疗效。方法对31例鼻腔鼻窦内翻性乳头状瘤鼻内镜下先切除鼻腔、筛窦、蝶窦、额窦口、上颌窦口肿瘤及其基部黏骨膜,侵及上颌窦者经柯陆径路内镜下于骨膜下分离切除肿瘤。结果术后随访2.5～7年,平均3.8年。治愈28例(90.32%),复发3例(9.68%)。结论鼻内镜对鼻腔鼻窦内翻性乳头状瘤疗效确切。切除肿瘤及其基部黏骨膜,对侵及上颌窦者联合柯陆径路内镜下治疗,可有效预防术后复发。

31 cases of rhinosinusal inverted papilloma treated with nasal endoscopic surgery

Objective To study the methods and therapeutic effect of nasal endoscopic excision of rhinosinusal inverted papilloma. Methods Nasal endoscopic excision of papilloma located in nasal cavity, ethmoidal or sphenoidal sinuses and ostia of frontal or maxillary sinuses as well as their basal mucoperiosteum was performed in 31 patients with rhinosinusal inverted papilloma. And then the tumor involved in the maxillary sinus was totally dissected under periosteum via Caldwell-Luc approach. Results All the patients were followed up for 2.5 to 7 years with an average duration of 3.8 years. 28 cases were cured (90.32%), and 3 got relapsed (9.68%). Conclusion Nasal endoscopic surgery is effective for the treatment of rhinosinusal inverted papilloma. Dissection of the tumor and its basal mucoperiosteum under endoscope, combined with the removal of tumors involved in the maxillary sinus via Caldwell-Luc approach, may prevent the recurrence of the tumor.
