急性冠脉综合征合并糖尿病患者血浆hs-CRP、BNP与左室功能关系的研究

摘    要:目的:探讨血浆高敏C反应蛋白(hs-CRP)及B型脑钠肽(BNP)水平与合并2型糖尿病的急性冠脉综合征患者心功能的相关性。方法:119例急性冠脉综合征,分为单纯急性冠脉综合征患者64例,合并2型糖尿病患者55例;血浆hs-CRP、BNP水平采用酶联免疫吸附法(ELISA)检测,左心室射血分数(LVEF)及左心室舒张末内径(LVDD)采用彩色超声多普勒测定。结果:LVDD及血浆hs-CRP、BNP水平合并2型糖尿病的急性冠脉综合征组显著高于单纯冠心病组,而LVEF显著低于单纯急性冠脉综合征组,两组间比较差异有统计学意义(P<0.05);血浆hs-CRP、BNP水平与LVDD呈正相关,与LVEF呈负相关(P<0.01)。结论:血浆hs-CRP和BNP水平对于评价合并糖尿病的急性冠脉综合征患者的心功能有一定的意义。

Abstract:Objective To investigate the relationship between high-sensitivity C-reactive protein(hs-CRP) or brain natriuretic peptide(BNP) and left ventricular dysfunction in acute coronary syndrome(ACS) patients with complicating type 2 diabetes.Method Left ventricular end-diastolic diameter(LVDD) was calculated by using colored doppler echocardiograph for detecting LVEF and relevant indexes combined with surface area in 64 cases of simple ACS and in 55 cases of ACS with complicating type 2 diabetes.Results In the group of ACS with complicating type 2 diabetes,LVDD and plasma hs-CRP and BNP were higher and LVEF was lower significantly than those in the group of smiple ACS.The difference was statistically significant as follows:The partial correlation analysis showed that hs-CRP or BNP were correlated to LVDD(P<0.01),hs-CRP or BNP were related negatively to LVEF(P<0.01),and hs-CRP was correlated to BNP(P<0.01).Conclusion The changes of plasma hs-CRP and BNP are significant in the evaluation of left ventricular function.
